Instructions Please answer the questions listed in each slide in summary format and provide any other relevant information pertaining to the slide heading as necessary. You do not need to answer each question literally or line by line, just be sure that the information included in the deck achieves the listed objectives. If you cannot answer some of the questions, that is ok. Although only 8 slides are listed, feel free to make your deck as long as necessary to convey all the appropriate information regarding your innovation. Visuals, Data, Gantt Charts, etc. are highly encouraged.

Objective: Describe the Market Questions to Answer/Consider: Size the market using incidences of the condition or procedure, revenues, and any other measures. If you don’t have published numbers, can you estimate based on other facts or reasonable assumptions? Who are the major competitors in the space? How is the market served now? Describe the existing products / solutions. What is the opportunity, unmet need, or problem to solve in this market? What is the value of solving it in terms of survival, quality of life, $ savings, etc.? What other products/solutions are currently in development that seek to address all or part of this opportunity/need? Are there other dynamics in the market relevant to your project?

Objective: Describe Technology/Solution/Product Questions to Answer/Consider: How does it work? Who will use it? (Include pictures if you have them) How does it serve the market opportunity / need? How does it do this better than other existing or proposed solutions? (include any illustrations of performance if available) Who are the decision makers who will “adopt” or not? How will they decide? Have you talked to any of them? How will does your solution fit with the way things currently work? How will you address areas where your solution doesn’t easily fit? What is the value proposition? (cheaper, faster, better) What is the IP position and status? What are the key filings?

Objectives: Describe Technology development Pathway; Summarize Data and Achievements to Date Questions to Answer/Consider: What is the path to prepare the technology/product for market? Include the major research, testing, and regulatory decisions. Use a chart or timeline if that makes sense for your project. What steps in the process have you already achieved? Provide evidence and visuals if you have them. Any other sources of technology validation to date?

Objective: Propose Milestones, Budget, and 12 Month Plan Questions to Answer/Consider: What work do you propose to do with the N.XT investment? Please provide milestones. What are the success criteria for the work? What will is cost to fulfill the proposed milestones? (provide detailed budget, up to 200k max) Where does it fit in your technology development pathway? What are the main activities, who will do them, what is the budget, how long will it take? Gantt Charts or visuals will greatly enhance the application

Objective: Describe the Commercialization Pathway and Likely Sources of Funding Questions to Answer/Consider: If the proposed work succeeds, what happens next? What is the model for commercialization? (e.g. “we will try to get past 510k then license or heavily partner”; “based on hitting this milestone we will try for angel money and produce and sell ourselves”) What kinds of private support will the project attract? Have you spoken to anyone in these categories? If so, what did they say?

Objective: Describe the Team Questions to Answer/Consider: Who are the investigators? Advisors? Among all the collaborators, who is leading the project? Is there anyone among the collaborators, or outside advisors, who is interested to manage the project once it becomes commercial?
