Community ENT service Dr John Rees at Woodbridge Hill Surgery Additional capacity required Short waiting times & GPwSI/Clinical Nurse Specialist with rapid access to Consultant if required Community based clinics around G&W Appointments on C&B for easy RSS access & activity monitoring Provider to host the service being actively sought 1 st April start as stated in QIPP 2014/5 plans

Inclusion Criteria - Adult conditions (17 years and above) Otalgia Nasal polyps Aural Toilet Chronic Rhinitis Otitis Externa Recurrent epistaxis Otitis MediaGlue ear Impacted ear wax removal/failed syringe/contraindication to syringe Snoring Perforation of eardrumGlobus (no suspicion of cancer intermittent) non smoker, no weight loss) Mastoid cavities requiring evaluation and toileting Intermittent hoarseness >3 weeks Dizziness/vertigo if known BPPV Cough, catarrh, throat clearing where laryngopharyngeal reflux may be present Nasal blockage ± facial painRemoval of foreign bodies Voice disturbances Loss of sense of smell Catarrh and post nasal drip Paediatric Conditions (16 Years and below) Nose Bleed (Epistaxis) Glue Ear (age 3 and above)
