Why?  The main reason that Lady Macbeth is at fault for the events of the play, is simply because she is the one who pushes, and in some cases forces, Macbeth to make use of the witches prophecy.  How?

Questions his manhood  “art thou afeard to be the same in thine own act and valour, as thou art in desire”  In English-are you afraid to be to be the same person in your actions and bravery as you are in your desires.  “be so much more than a man”  In English- He has to be brave enough to kill the King.

Questions his word  “Was the hope drunk wherein you dressed yourself? Hath it slept since?  In English- was the ambition you had left you?

Encourages him to be Ambitious  “Greater then both by the all-hail hereafter”  In English- according to the promise of the future.  “This ignorant present”  In English- Your letters that have allowed me to see the future.

Questions his love  “From this such I account thy love”  In English- considers his love to be as weak as his ambition.
