6 of 11 Sam’s REED mistakes Student information not completed. This is essential to keep records of the student accurate. Completing the section fully will help staff file records correctly

7 of 11 Sam’s REED mistakes No general education teacher participated. The general education teacher is a required IEP Team member and is expected to participate in the REED. The teacher would be expected to contribute on student progress in the general education curriculum and any needs the student may have in the general education classroom. The general education teacher may have observations and other performance data to share.

8 of 11 Sam’s REED mistakes Data sources is a “person” Data sources may require more detail. For example rather than the speech teacher the data source may be the service logs that record attendance, progress on goals and objectives and other information

List relevant items that more specifically describe the student such as: 95% attendance, raises hand to voluntarily participate, completed spelling tests at 85% average over 15 weeks…

11 of 11 Sam’s REED mistakes Notice of sufficient data did not include the reason. It is required that the team describe why no further evaluation would be needed for the student to continue being eligible for special education instruction.
