Beaver County Continuum of Care DINA CIABATTONI, LSW CONTINUUM OF CARE COORDINATOR BEAVER COUNTY

Continuum of Care The purpose of the Continuum of Care program is to: o promote communitywide commitment to the goal of ending homelessness o provide coordination, funding, and support for efforts by nonprofit providers and local governments to quickly rehouse homeless individuals and families while minimizing the trauma o optimize self-sufficiency among individuals and families experiencing homelessness

Who makes up the Beaver County CoC? 8 HUD funded supportive housing programs for people who are homeless and have a disability 2 emergency shelters (one for men, one for women who experience domestic violence) Coalition made up of 150 diverse social services & housing providers who offer services to support the complex needs of people who experience a housing crisis. Meets monthly and is open to the public The Steering Committee serves as the HUD-designated primary decision-making group and oversight board for the CoC. Meets quarterly.

Point In Time Survey In our most recent Point In Time survey, we counted: 172 people (constituting 74 households) who were experiencing a housing crisis that day. A housing crisis can mean living in: a motel, a place not meant for human habitation, or temporarily living with family and/or friends (coach surfing)

Point In Time Survey

PIT Data (continued) 346 homeless people were housed in supportive housing programs that night Because self sufficiency is a CoC goal, safe & affordable housing options for homeless people to transition into upon exiting a supportive housing program are an imperative piece to them maintaining stable housing.
