The process of secondary school transfer for pupils with an EHC Plan is different to those children who don’t have an EHC Plan. Children who have an EHC.

TAC: TAC meeting to be arranged. Plan, Assessment and Review Officer to attend (CO). Prepare Request : Lead Practitioner will collate all necessary evidence and opinions. My Plan and My Plan + documentation and reviews will be considered, along with supporting evidence of interventions, strategies and provision following recommendations from external agencies. The impact of these will be discussed and PARO will suggest any other interventions/advice needed. Incomplete or inconclusive information may be challenged. The PARO will advise on further work at My Plan + or an application for Assessment is most appropriate. Lead Practitioner to be agreed. The agreed Lead Professional will collate all information, advice and reports for submission with the Request for Assessment Form, checklist, My Plan + and Review documents. This information will be sent to the Plan, Assessment and Review Team (Shire Hall) if agreed with the PARO. If further evidence needs collation it will be determined on a case by case basis whether another TAC meeting is required. Requesting Assessment towards EHCP process

Week 0: Request received TAC submit My Plan + documents to inform assessment process Week 5: EHCP Request Panel Requests for information will be sent from Plan, Assessment and Review team to all relevant professionals and agencies inc. Education, Health and Social Care, giving them 4 weeks to respond. A Multi Agency Panel will meet to discuss the case and decide whether or not to continue to a EHC Plan Assessment If Assessment is agreed, further evidence will be gathered to inform Resource Panel where the request will be considered by week 12. If Assessment is not agreed the parents/carers/CYP will be advised of this and of the rationale. Support at My Plan + and single agency level will continue as determined appropriate by those agencies involved. 20 week Statutory Assessment process

Week 6: Parent/carer, CYP and all parties advised of Panel decision. Opportunity to confirm or update practitioner/parent/carer/CYP reports and opinions is given. Week 12: All reports to be received by this date. Practitioners will be asked to confirm that their evidence/opinion remains the same or to provide updated information to be included in any future My Plan + or EHCP. The PARO may be in communication with all/some parties to liaise over potential provision etc through a future My Plan + or EHCP. The PARO will prepare and present the Request to EHCP Resource Panel and communicate decisions as appropriate. 20 week Statutory Assessment process

Week 13: EHCP Resource Panel Week 16 – Issue Draft EHCP If an EHCP is agreed Service Leads or their delegated Officers will agree and sign off indicative resourcing. Lead Practitioner to arrange a TAC meeting to comment on/agree/suggest amendments to any proposed EHCP. Where possible PARO will attend. School preference to be ascertained and Transport team to be informed. The Child/Young Person, Parents/Carers have 15 days to make any further requests/suggestions/comments regarding the EHCP. School preference to be confirmed and, following consultation, the school/setting have 15 days to assess whether or not they can meet the needs of the CYP as outlined in the EHCP. Week 20 – Final Plan published
