Next Steps (ACT) Continue to have hospital staff make support calls Review readmissions 30 and 180 days from Discharge Utilize Peer Specialists/Clubhouses Review each client’s case

IMPACT 1. The goal was 3% or less re-hospitalizations. Clients that choose to participate 2 clients were readmitted (2.7 %). Of the clients who choose not to participate 10 Clients were readmitted. (9%) 2. Clients feel support upon discharge. 3. Enhance staff morale. 4. Better understandings of other departments. 5. Increased emphasis on hand offs and crisis planning.
