2012 ( ) Annual Performance Report (APR)

APR Overview  The APR is written to report on our state’s performance toward the Indicators outlined in the Colorado State Performance Plan (SPP)  SPP sets targets and outlines improvement activities  APR includes actual data as it compares to targets in SPP and progress on SPP improvement activities  The Office of Special Education Programs (OSEP) requires States to publicly report APR data on local early intervention programs

APR Overview Cont.  The OSEP also requires States to make status determinations for local early intervention programs based on APR compliance data and other information, including timely correction of noncompliance  The OSEP also makes status determinations for States based on performance on APR compliance data, which includes timely correction  Colorado’s status determination was “Meets Requirements”

APR Overview Cont.  2012 APR is reporting on the last year of 2005 – 2012 SPP  Beginning work on a new SPP which will include existing Indicators plus a new Indicator 11 State Systemic Improvement Plan

1: Percent of infants and toddlers with IFSPs who receive the early intervention services on their IFSPs in a timely manner (within 28 days). Actual Target: 97.49% : 96.29% APR FY Indicator 1: Target 100%

2: Percent of infants and toddlers with IFSPs who primarily receive early intervention services in the home or community-based settings. Actual Target: 99.78% : 96.29% APR FY Indicator 2: Target 95%

APR FY Indicator 3A1: Target 68% 3A2: Target 75% Positive Social Emotional Skills 3A1: Percent who exited the program with substantial growth Actual Target: 72.04% : 69.60% 3A2: Percent who were functioning within age expectations at exit Actual Target: 66.76% : 73.20%

APR FY Indicator 3B1: Target 62% 3B2: Target 67% Acquisition and Use of Knowledge and Skills 3B1: Percent who exited the program with substantial growth Actual Target: 79.14% : 74.50% 3B2: Percent who were functioning within age expectations at exit Actual Target: 48.66% : 59.20%

4A: Percent of families participating in Part C who report that early intervention services have helped the family: Know their rights. Actual Target: 92.00% : 95.00% APR FY Indicator 4A: Target 85%

4B: Percent of families participating in Part C who report that early intervention services have helped the family: Effectively communicate their children’s needs. Actual Target: 94.00% : 95.00% APR FY Indicator 4B: Target 88%

4C: Percent of families participating in Part C who report that early intervention services have helped the family: Help their children develop and learn. Actual Target: 95.00% : 94.00% APR FY Indicator 4C: Target 85%

5: Percent of infants and toddlers birth to 1 with IFSPs. Actual Target: 1.0% :.91% APR FY Indicator 5: Target 1.05%

6: Percent of infants and toddlers birth to 3 with IFSPs. Actual Target: 3.0% : 2.79% APR FY Indicator 6: Target 2.5%

7: Percent of infants and toddlers with IFSPs for whom an evaluation and assessment and an initial IFSP meeting were conducted within Part C’s 45-day timeline. Actual Target Data: 98.94% : 99.05% APR FY Indicator 7: Target 100%

8A : Percent of all children exiting Part C who received timely transition planning to support the child’s transition to preschool and other appropriate community services by their third birthday including: IFSPs with transition steps and services. Actual Target Data: 99.17% : 99.73% *Draft as of 9/23/13 APR FY Indicator 8A: Target 100%

8B : Percent of all children exiting Part C who received timely transition planning to support the child’s transition to preschool and other appropriate community services by their third birthday including: LEA notification. Actual Target Data: 98.90% : 100% *Draft as of 9/23/13 APR FY Indicator 8B: Target 100%

8C : Percent of all children exiting Part C who received timely transition planning to support the child’s transition to preschool and other appropriate community services by their third birthday including: Timely transition conference. Actual Target Data: 96.04% : 98.37% APR FY Indicator 8C: Target 100%

Individual Performance Profiles  CCB Individual Performance Profiles were sent out for comment. Comments are due back by December 13 th  Performance Profiles meet the requirement for publicly reporting data for local early intervention programs  Include all of the required measurements of reporting and comparisons between programs and compared to state targets  Status determinations are also publicly reported, although not required

Provider Database/Child Outcomes  Contact Connie Fixsen ( ) or to transfer a child’s COS record from one CCB to another. The sending CCB should request the transfer.  If it is known that the child has received services prior to coming to your CCB and has a rating, a transfer must be requested.  Adding the child without first requesting and receiving a transfer creates duplicate records.
