Commissioning priority of all Lancashire Clinical Commissioning Groups and Lancashire County Council Central Lancashire strategy and action plan Forms part of the contract monitoring framework Work with dementia adviser service to implement “living well and planning for the end of life” Six steps training programme – social workers – Residential and nursing homes – Domiciliary care workers

What else is happening Provider audit to influence future training within the sector UCLAN/ Age UK/ knowledge sharing partnership project developing a volunteering model to support people at the end of life Work with help direct and the customer service access service to ensure local and national supports including bereavement services are listed Link to the work being undertaken by the community connectors Included on the hospital passport Marking dying matters week
