2 Prosper Promoting Safer Provision of Care for Elderly Residents 2 Funded by The Health Foundation - Closing the Gap in Patient Safety First time a Social Care scheme has been chosen Essex County Council & UCLPartners working in partnership with Essex Residential Care & Nursing homes Overarching aim to reduce the number of emergency hospital admissions Focus on Prevention - reducing the prevalence of falls, pressure ulcers and Urinary Tract infections across care homes. 63 homes participating with a further 10 starting in November 2015

3 Prosper Methodology 3 Building staff capability through education in quality improvement methodologies – PDSA cycles (plan do study act) of small tests of change for continuous improvement Using data measurement over time to inform improvement cycles – moving homes away from feeling that data is only used for negative reasons Changing staff culture & behaviour on safety from being reactive to proactive & preventative

9 Implementation 9 On spot debriefs following falls so that staff can talk through what's happened Gizmos and gadgets such as mirrors and skin blanchers Get homes involved with national events but on a local level (NH Week) Regular visits to the home to support Toolkits for staff and managers that will be used!

10 Education, Education, Education!! 10 Simply put – you don’t know, what you don’t know! PROSPER Champions days Diabetes Project in North East Essex. Essex wide Catheter care management package put together by CCGs, Community Health Providers and ECC Essex Wide Pressure care training package put together by CCGs and ECC 8 weeks falls prevention training programme Dyshasia awareness training – joint event with homes and hospitals being arranged Result: Staff are researching information and training on their days off and bringing it back into the home!

11 Results – One Year On 11 Interim evaluation has reported a change in staff culture across all homes participating. Improved data recording, capturing information previously not recorded – Falls, UTI’s, pressure ulcers, hospital admissions Homes are using data to inform proactive approaches to prevention No statistical significant change at this stage – challenges in collecting historical data

12 By Products 12 Integrated working between Health, Social Care and Care Homes Homes working together and building up better resilience and sustainability Consistent approach across CCG boundaries Sharing ideas Influencing future commissioning within Essex both at an internal level as with as with CCG partners
