What, Why and How (using i2i Tracks) March 14, 2016

Presentation Goals Explain dashboards Balanced Dashboards Discuss why dashboards are useful Show how to set them up in i2i Tracks Demonstrate how to use dashboards as a component of a QI plan

What is a dashboard A representation of data that has been collected for all members of the clinical team Designed to display data in an intuitive and simple format

Why are dashboards useful? An effective tool to shift organizational culture. They provide a foundation to recognize successes, understand emerging problems and make changes in response to these problems. They provide insight into clinical data and can be used to determine program viability, identify areas for improvement, monitor change and improvement, and point out where reallocate resources may be necessary. Dashboards display data in a useful and effective way. They are foundational to recognizing successes, identifying and responding to emerging problems.

Why use dashboards? To improve: Participation with government and other regulatory reporting requirements (UDS) Payer and accreditation organization requests for information and standards compliance (PCMH, HEDIS) Reporting to administrative boards Monitoring of patient safety and quality of care provided Dashboards are a recommended means of assessing and overseeing an organization’s performance based on quality of care metrics; determined from internal and external sources

Goal of using dashboards …track data, improve operational and clinical performance, promote accountability and transparency and shift organizational culture

Dashboard Basics Determine Key Objectives for display. Should tie back to your QI Plan and stated goals and objectives for clinical quality improvement. Consider using a Balanced Scorecard approach that displays clinical quality, operational efficiency and financial productivity indicators. Further the indicators are weighted based on organizational goals. This approach is particularly useful to avoid emphasizing one element of quality over another. Develop Key performance indicators/measures for areas related to the organization’s QI Plan (indicators can be structure, process or outcome – focused and should be balanced as described earlier) Fields, S. and Cohen, D. (2011) Performance Enhancement Using a Balanced Scorecard in a Patient-Centered Medical Home. Family Medicine 43:10 Nov./Dec. 2011

Dashboard Basics Develop a scorecard; monitor and audit over time. Set expectations with teams Provide regular feedback (preferably quarterly) to Care Teams. Use quality management principles to identify areas that need a closer look; exceed or below benchmarks Select best practice benchmarks either internally or externally Fields, S. and Cohen, D. (2011) Performance Enhancement Using a Balanced Scorecard in a Patient-Centered Medical Home. Family Medicine 43:10 Nov./Dec. 2011

Challenges Developing an effective QI plan and process that includes Key Performance indicators based on regulatory, accreditation and organizational goals for clinical quality, operational and financial performance. Ongoing development and maintenance of internal (EHR and i2i Tracks) quality reporting systems Effective collaboration that leads to identifying core measures that support the QI program and plan for the organization

Dashboards in Tracks Can be displayed on Tracks Today Screen Can be viewed from within iTi You can select which dashboards display Clinical dashboards can be viewed in a variety of formats and by default (PCP) provider, rendering provider, location or financial class Dashboards can also be customized within Tracks Dashboards can also be created by running Population Analytics reports from i2i
