Chronic (on-going) as well as acute (emergent) conditions which meet enrollment criteria are enrolled. Many conditions have an algorithm or formula to determine status.

Service member is more mission ready when Family needs are met.

With privatization of housing – must meet the Fair Housing Act. Family member’s needs are considered with documentation of medical needs and when appropriate housing is available.

The physician ONLY completes the required EFMP enrollment paperwork when requested by the Family member. He/She may refer for pre-screening through the hospital EFMP Case Coordinator/Special Needs Advisor.

Again, the Family member must provide EFMP enrollment paperwork and request the provider to complete for any significant change in condition or on/before the 3-year enrollment anniversary.
