2 Process and Outcome Measures Example AIM: To prevent Ventilator Acquired Pneumonia. (Outcome Measure). Measure: Number of patients with a diagnosis of pneumonia who are on ventilators (and were not admitted with pneumonia). Changes: There are 5 different changes that will be implemented and therefore 5 process measures: –head of bed elevated, stress ulcer prophylaxis, DVT prophylaxis, oral care, and sedation management.

3 These are the five processes that will be monitored (process measures) since they are predictors of ventilator acquired pneumonia.

4 Ventilator Management Dashboard Process Indicators – Real-time system with alerts (red, green, yellow flags) indicating compliance with the Ventilator Management Bundle.
