Term Assignment-Case Study You are working alone or with a partner (no more than 2) You have been given a Case Study by your instructor These are ‘real-life’ people in your case study We need to be sure that you know what is expected of you when you hand in your final ‘product’

Who are you in relation to the Case Study? You do not work for any particular agency You are the “Social Welfare Angel” You can connect your client to any agency You are not confined by confidentiality or legislation.

Outline requirements… No less than two full pages Point form Title page No plastic covers! Stapled in the corner 12 font Double spacing Correct spelling and grammar

Our aim in Human Service… Human service involves the transformation of knowledge into practice. The aim of human service is to help people become more empowered so they can function more effectively It is important in our future careers as Criminal Justice Professionals that we are able to refer clients to the appropriate community and social services.

To Achieve our Aim… We will apply a generic, formal, systematic, and scientific set of procedures. This problem-solving process is commonly referred to as the…. “planned change process”

PLANNED CHANGE PROCESS You must use this procedure when examining your case study. The planned change process has 5 phases. Each phase must be addressed in both the brief outline and the written assignment that you hand in.

1. Intake There is only one question during this phase. Would this client benefit from social welfare involvement in their life? If the answer is ‘Yes’ – proceed to Phase 2 Explain briefly why you answered yes or no Be aware that you must answer this question regarding every relevant person identified in your case study!

2. Assessment The ‘investigation’ phase This is where you collect information from a variety of sources—the more sources the better! Who would you talk to in an effort to collect information about the client? A good investigator collects information from as many sources as possible These are know as “collateral” contacts This investigation is to determine what the client’s true situation is?

3. Planning As the human service professional you decide what needs to be changed. You will discover this through your previous Assessment phase Perhaps it is a behaviour, an emotion, a pattern, or environmental situation In a ‘real-life’ situation the client would be involved by asking their opinion on their situation and changes that need to be made It is always best if the client helps in the development of the “plan” This is where you identify the NEEDS of the client, ie: education, employment, housing, etc.

4. Contracting This is where you link the NEEDS (identified in Phase 3 --Assessment) to the services. What agencies or services will you link the client up with? Here is where you should outline clearly your goals and objectives of the needed change What types of strategies will you use to put these into effect? How will you involve the client? How will you get the client/s to participate in the plan?

5.Intervention This phase involves putting the plan into action Exactly what agencies/services are involves? How often do they connect with the client? Who will monitor the client? Remember you are a social welfare “angel” and CANNOT monitor the client—Who will do it? When do you know the goals and objectives have been accomplished? When will you know that services can be discontinued?

Term Assignment Evaluation Please refer to the Term Paper Rubric posted on WebCt. The following elements will be evaluated: Cover Page Introduction Body Citations Conclusion Bibliography Mechanics Comprehensive
