1 Facilities Incident Notification

2 Reporting Requirements All employees are required to report immediately any incidents, accidents, or major equipment failures that have no redundancy and that have a direct or indirect effect on science or the site as soon as they become known. This includes injuries. This process is designed to: Promote employee safety throughout LBNL Minimize disruptions to the laboratory community Maximize response time for failing equipment Aid Facilities Management to meet reporting requirements

4 Report Immediately - Don’t Delay Relay what information you have at the moment – Describe: What – The event or condition Where – Building, floor, room, road, etc. When – Day, date, time, etc. – Don’t wait to see how things “develop ”

5 What Do We Report Any failure of a service, system, and/or utility that has no redundancy and has a direct or indirect effect on science or the site All injuries other than minor first aid

6 Examples Some examples for reporting – All injuries including all electrical shocks (not minor first aid) – Motor vehicle accidents – All permit violations (Penetration, LOTO, Environmental, Hot work, Fall Protection, etc.) – Utility failures (Electrical, Water, Natural Gas, etc.) for which there is no redundancy – Significant equipment failures – Hazardous energy, Hazmat, or shipping violations

7 – Building Evacuations – Emergency Situations (fires, explosions, toxic gas alarms or releases) – Chemical spill or exposure (includes radiological) – Water into storm drains – Minor safety hazards must still be corrected and/or reported but not through this process – This policy does not replace “Stop Work” for imminent danger Examples

9 After Hours-Who Ya Gonna Call Between 4 pm and 7:30am Notify the lead Plant Maintenance Technician Call ‘911’ first if appropriate 510-486-5481 Don’t wait to see how things develop-call

10 What Happens Next Work request will take and relay information Fact finders are dispatched - Promotes appropriate and rapid deployment of responders, or repair teams Management notified - Management team makes DOE notifications as necessary
