4 SCENARIO At 8:15 on a Saturday morning, staff at your facility notice that the fire alarm is going off. Upon arrival by the Fire Department, it is determined that the IT Server Room overheated and a minor fire occurred. Due to fire, smoke, and water damage, your IT Server Room is down for 96 hours.

5 QUESTION 1 With the IT Server Room down, what are the three most critical functions affected by this? Do you have back up systems in place to deal with those critical functions? How familiar is staff with those back up systems?

6 QUESTION 2 Would your Incident Management Team be notified of this incident? Or just the on-call Administrator? Is your calling tree up to date?

7 QUESTION 3 How is information regarding this incident relayed to staff? For Long Term Care Facilities, would you notify your residents of the incident? What would the communication entail? (ie nature of incident, what systems are down, anything else) How often would your facility provide updates? Review internal communications processes

8 QUESTION 4 If this incident occurred on a hot day that coincided with a big race in town, would your facility notify EMS at all? Would any other external organization be contacted due to this incident? Who makes the decision to contact external organizations? Review external communications processes

9 QUESTION 5 When the IT Server Room is operational again 96 hours later, what systems are the most important to bring back online first? Who, or what current plan, determines that?

10 QUESTION 6 During the duration of the incident, would there be any needs from the Coalition or neighboring healthcare facilities? If so, what would these requests potentially entail? Who would your facility call first? What does the communication process look like?

11 CONCLUSION Have you identified any gaps in your plans from this exercise? If so, what are the gaps and what training or education would be beneficial? Any other Comments?
