3  29-year-old woman, without specific antecedent  Delivery in february  + 14 Kg during pregnancy  Numbness in the hands (L > R), worsening at night and at rest, since month 7 of pregnancy  The numbness was less pronounced after delivery; but, the patient reported left finger stiffness (residual oedema ?) and a sensation of weakness in the left intrinsic hand muscles (thumb abduction for instance)  Since one month (april), the patient experienced pain in the left wrist over the ulnar or radial side; the numbness has disappeared

5 Examination was normal on right upper limb and both lower limbs On the left upper limb :  No reflex change  Phalen’s sign : +, Tinel’s sign : - Carpal compression test : pain without paresthesia  Decrased pin-prick sensation over the thumb and the ulnar aspect of the hand No loss of touch sensation  Adson’s test : + Chandelier : pain in the thumb No tender spot or Tinel’s sign on supraclavicular area  Muscle weakness : abductor and extensor pollicis and flexor carpi (C8D1)

6 X-ray of the cervical spine : normal, no cervical rib X-ray of wrists and hands : normal Biological findings :  No inflammatory syndrome  Blood cytology and ions : normal  Immunology: - no gammopathy - no antibodies against the peripheral nervous system - normal C3, C4 complement components - rhumatoid factor : 43 UI/ml (0-11) - ANA : + (speckled) HEP.2 cell titer : 1/160 -anti-DNA : - - ANCA : -
