2 Origins of Hospice on PEI The Island Hospice Association was incorporated in July 1985 and changed its name to the Hospice Palliative Care Association of Prince Edward Island in 2001. Today, we are known as Hospice PEI.

3 National body advocating for hospice palliative care services on the National level CHPCA’s Vision is “That all Canadians have access to quality end-of-life care” There are more than 300 existing hospice palliative care associations across Canada

4 What is Hospice PEI? Province-wide volunteer organization celebrating 30 years of assisting Island families Registered charity and relies solely on its own fundraising efforts and public donations In 2014 our trained volunteers provided over 8,000 hours to over 500 Island families

5 What Support do Volunteers Provide? Support to palliative care clients in all care settings including long-term care, hospitals, the client’s home & the new Provincial Palliative Care Centre Support to committees of Hospice PEI as Board Members, Event support, Public Relations, Grief Support, And Chapter Advisory

6 Vision and Mission of Hospice PEI People across PEI accessing the support, care, and bereavement services they want, in the setting of their choice, whenever a life- limiting illness affects them -- from diagnosis to the time following death. Hospice PEI is a volunteer- driven organization assisting people in PEI by preparing, supporting, and caring for those affected by a life- limiting illness before and after death.

7 CORE VALUES Clients are the focus of the work we do We respect each other, our clients, our partners, our public, and ourselves We are caring and compassionate in all our actions We work collaboratively within our organization and with others Decisions made at all levels of the organization are informed and evidence-based

8 Hospice PEI Strategic Plan To accomplish Hospice PEI’s Mission, the governing Board has set strategic directions and specific goals to focus the work of the organization for the next three years.

9 OPTIMAL CARE FOR CLIENTS Increase the number of clients receiving the care they want, in the setting of their choice, with an added focus on underserved populations in long term care facilities.

10 Volunteer Development Hospice PEI’s volunteers and support staff are trained to a provincial standard Volunteers from within other organizations can access Hospice PEI training Hospice PEI’s volunteers represent our diverse population. Hospice PEI’s active volunteer base increases and covers a diverse range of roles

11 Financial Stability Goals Increase financial stability by increasing direct donations, corporate multi-year donations, and program funding – based on a multiyear funds development plan Establish a Board-driven planned giving program

12 Public Awareness, Visibility, and Education Goals More people contacting Hospice PEI for services More people receiving information about hospice and palliative care

13 WHO CAN ACCESS OUR SERVICES Our services are free of charge and referrals are accepted from family members, friends, health professionals, home care workers, the patient him/herself and by Hospice Coordinators
