2 At the beginning of every month, RN’s whose DOH is the next month (e.g. in June, all July DOH’s), receive a packet of information and an e-mail regarding the evaluation process. The due date for return of all completed materials to the manager/supervisor is 3 weeks from distribution of packet.

Enclosed is an overview of the Performance Evaluation Process, a Peer Feedback Tool, the Performance Ratings Guidelines and your individual documentation audit results from the previous year. You have also been sent an electronic copy of the Framework Self-Evaluation form for you to complete after reviewing the completed peer feedback and individual documentation audit results. If you prefer a paper copy, please print one from the e-mail or request a copy from Tina. The due date for submission of your completed evaluation to me is_________________. If materials aren’t submitted within two weeks of the established due date then management may proceed with completing the evaluation process. Please see the enclosed Performance Evaluation Process sheet for complete details. Thank you in advance for timely completion of your part in the evaluation process.

5 E-Mail Hi ______, Your annual evaluation is due in February and I have put a packet of information in your boxes re: the process. THE SELF EVALUATION FORM FOR LEVEL C is ATTACHED TO THIS E-MAIL. You may either complete the self-evaluation form electronically and e-mail it back, or print it off and complete it in paper format. Included in your packet is your individual documentation audit results from the previous year. Please review the data, congratulate yourself for your areas of strength, identify any problem areas, and incorporate this feedback into your self-evaluation as necessary. As stated in your packet, the due date for submission of your completed evaluation is January 27th. If the materials aren't submitted within 2 weeks of the due date, we may proceed with completing the evaluation process. (Your signature on the form certifies that you have reviewed the policy). The completed disclosure form should be submitted with the rest of your evaluation packet. 6 After EE submits Completed Evaluation Materials to Manager/Supervisor If self-evaluation is submitted electronically: Peer feedback is added, the Manager Evaluation portion is completed If self-evaluation is submitted on paper: An electronic evaluation form is created. The peer feedback and self-evaluation is added and manager portion is completed. After Manager Portion is Completed: A meeting time is set up with employee to review completed evaluation.

8 Barriers to Process <10% of staff complete self-evaluation electronically Difficult to access H drive. Difficult to find time to complete during work time. Easier to print and complete on paper. 16 page Peer Feedback form-Distributed to 3-5 peer reviewers Forms sometimes not returned to EE in timely manner. Negatively impacts staff’s ability to meet deadline.
