4   1 cup milk  1 cup fruit/vegetable (full strength juice)  2 grain/bread  2 meat/cheese Dinner

5   Any two of the following:  1 cup milk  1 cup fruit/vegetable (full strength juice)  2 grain/bread  2 meat/cheese Snack
