7 Washington State Department of Health These students are in compliance with the requirements  Fully immunized = vaccinated according to national recommended immunization schedule  Immunity by lab test (documentation required)  History of chickenpox  Exemption on file

8 Immunization Status Complete (fully immunized) No further follow-up required Conditional (temporary status to get immunizations) Follow-up until fully immunized Exempt (COE on file) No further follow-up required unless temporary medical exemption Out of compliance (missing immunizations) Must be excluded according to the law

10 School Immunization Requirements

11  5 doses required for K-12 th grade Exception: 4 doses required if 4 th dose given >4 th birthday Washington State Department of Health Immunization Requirements - DTaP

12  1 dose required for 6-12 th grade IF student is 11 years of age or older Washington State Department of Health Immunization Requirements - Tdap Exception: Tdap given between 7-10 years of age counts for the Tdap requirement

13  DTaP: 6 years of age and younger  Tdap and Td: 7 years of age and older  If no DTaP when younger:  Tdap + 2 Td Washington State Department of Health Immunization Requirements – DTaP/Tdap/Td

14  3 doses required for K-12 th grade  K-8 th grade: Dose 3 must be given min age of 24 weeks  9 th -12 th grade: Dose 3 valid at 4 months of age Immunization Requirements – Hepatitis B
