2 Health Risk Assessments (HRA) Be proactive in managing your health! Identify individual risk factors and try to modify your lifestyle as appropriate. By completing a Health Risk Assessment you will also save money on your health insurance premiums effective January 1, 2016.

3 What is a HRA? It is a two-part process, including: 1.Biometrics screening, and 2.Online health questionnaire HRA’s are completely confidential and voluntary Annual process for all employees and spouses who are covered or will be covered under the MU Medical Plan, must complete each calendar year to receive a discount Both parts must be completed in order to receive a discount on 2016 health insurance premiums.

4 Why should I participate? Raises health awareness for the participant. Prioritizes top areas needed to maintain or improve health. 10% discount on your health insurance premium for the next calendar year The blood lipid results are obtained in five minutes. All information is confidential.

5 Who is eligible to participate? On a voluntary basis, all employees and their spouses are eligible to participate 2016 discount: For employees who have a spouse covered or will be covered by Marquette’s medical plan, the employee and the spouse must complete both parts of the Health Risk Assessment process in order for the employee to receive the 10-percent discount for 2016. If only one of the two complete the required steps, the employee will not be eligible for the 10-percent discount.

6 Biometric screening What does a biometric screening consist of? It is a simple finger stick test that will analyze your cholesterol, HDL and glucose. Blood pressure, pulse, height and weight are recorded. For the most accurate results a 12-hour fast is required prior to your screening. No eating or drinking (except for water and black coffee) Continue to take your prescribed medications as directed. The results are available in about 15 minutes. Y OU WILL NEED THESE RESULTS TO COMPLETE THE ONLINE HEALTH QUESTIONNAIRE. P LEASE SAVE THESE RESULTS.

Option One: Utilized your Primary Care Provider*  Visit your primary care provider between July 1, 2015 – November 20, 2015  Co-pays may apply  Fax the Primary Provider Form, available on the HRA Information website, to Aurora Health Care (Attention: Justin) at (414) 525-2580 HRA Information website *Be sure to keep a copy of the biometric screening in order to complete the online questionnaire

Option Two: Complete your screening free of charge at MU-sponsored Aurora offerings*  Schedule your free appointment between the following dates: Oct. 26, 2015 – Nov. 21, 2015  To schedule via phone, call (877) 765-3213 (Option #1)  To schedule online, go to www.aurora.org/apptregmarquette www.aurora.org/apptregmarquette *Be sure to keep a copy of the biometric screening results in order to complete the online questionnaire which will be available beginning October 26, 2015.

Option Three: Complete a comparable HRA at spouse’s employer*  Biometric Screenings must be completed during the following dates: July 1, 2015 thru November 20, 2015.  Fax your lab results from your spouse’s HRA screening to Aurora Health Care (Attention: Justin) at (414) 525-2580  In order to connect all documents, please make sure Marquette University as well as the MU employee’s name is prominent on any alternative form you submit. *Be sure to keep a copy of the biometric screening results in order to complete the online questionnaire which will be available beginning October 26, 2015.

10 Health Questionnaire What is the online health questionnaire? The second step of the HRA process is to complete an online questionnaire using the results from your biometric screening. This questionnaire will be available beginning Monday, October 26th, the start of the Annual Enrollment Period, through midnight on Monday, November 23, 2015. It is a simple self reporting questionnaire regarding your lifestyle, administered through the University of Michigan. It will help you identify risk factors. U PON COMPLETION OF THE QUESTIONNAIRE, PRINT A COPY OF YOUR RESULTS. K EEP FOR YOUR RECORDS AND AS VERIFICATION THAT YOU HAVE COMPLETED THIS STEP, IF NEEDED.

11 Health Questionnaire How do I complete the online health questionnaire? Go online to complete the questionnaire, using your biometric screening results. The online tool will be available Oct. 26 – Nov. 23 through the www.marquette.edu/benefits web site. DO NOT ENTER YOUR SS NUMBER WHERE IT ASKS FOR THIS!www.marquette.edu/benefits Please note: If you participated in the HRA process in the past, make sure to use the same Unique Identifier number and password to receive comparison results (i.e. 9-digit MUID number). NO DASHES or EXTRA ZEROS PLEASE
