WHAT IS MY WELLNESS PROGRAM? ▪ The EPC Wellness Program will provide you with tools, support and education about your health status and reward you for participating Step 1 Completion of your Personal Health Assessment (PHA) You receive $35 for completing Step 2 Completion of a Wellness Workshop You receive $65 for completing Step 3 Redeem your incentive dollars

STEP 1: PERSONAL HEALTH ASSESSMENT (PHA) A Personal Health Assessment is a confidential 15-minute questionnaire that asks you about your lifestyle habits; such as diet, exercise, stress level or smoking habits Once the questionnaire is completed, you will receive a detailed report that rates your health risks (Low Risk, Moderate Risk, or High Risk) and makes personalized recommendations about how to better manage your health, based on your risks An overall wellness score is also calculated and provided The wellness score begins with 100 points; points are then deducted depending on responses to questions

PHA answers are used to calculate a Low, Moderate, or High risk for each category of the PHA The main objectives of a Personal Health Assessment are to: ▪ Assess your health status and provide you tools and resources to enhance your health STEP 1: PERSONAL HEALTH ASSESSMENT (PHA)
PROGRAM OUTREACH ▪ If you have not signed up for a Wellness Workshop within 30 days of completing your Personal Health Assessment, you will receive an encouraging you to do so ▪ If you have not signed up for a Wellness Workshop within one week of receiving the initial , you will receive a phone call ▪ You will receive an once you are half-way done with your Wellness Workshop to encourage your continued participation ▪ You will receive a phone call after the completion of your Wellness Workshop congratulating you for the Workshop completion and to assist with providing you with any tools or resources that you need
A wellness workshop is an online program that provides self-paced education around a specific topic You will need to complete a workshop to be eligible for the $65 incentive. We encourage you to complete as many workshops as you like, only one workshop will be counted toward your incentive The workshop provides information necessary to make healthy changes and the guidance to take action in further developing and advancing healthy behavioral changes ▪ Articles, tip sheets, wellness tools and tasks STEP 2: WELLNESS WORKSHOPS

STEP 2: WELLNESS WORKSHOPS You are able to sign-up to take a wellness workshop by clicking the Signup button You may take more than one workshop at a time but, you must complete at least one in order to earn the incentive After signing up for a workshop, you can decide to switch workshops if another sounds more appealing

STEP 3: REDEEM YOUR INCENTIVE DOLLARS Earn up to $100 by completing the PHA ($35) and a wellness workshop ($65) Incentive dollars are paid out in the form of gift cards There are over 200 gift cards to choose from in the Hallmark Gift Card Center You can order multiple gift cards in varying amounts (e.g. $100 gift card or $25 and $75 gift once the PHA and Wellness Workshop complete, $35 gift card after PHA completion)

FREQUENTLY ASKED QUESTIONS Q: When does the program begin? A: The program will be available beginning October 6, Q: Who is eligible to participate in the program? A: This program is a free benefit for employees who are currently enrolled in the EPC medical benefits through Anthem or UnitedHealthcare and are the primary subscriber. Q: Do I have to participate in the program? A: No, participation is voluntary. However, by participating, you not only have the opportunity to learn more about your health, but also to earn incentive dollars. Q: Is participant privacy and confidentiality protected? A: Yes. Your individual results are completely confidential and are protected by HIPAA, GINA, and HITECH privacy regulations. Q: Will my results affect my insurance rates or eligibility? A: No. By law your results cannot be used to affect the cost of your insurance premiums or your eligibility for health insurance.
FREQUENTLY ASKED QUESTIONS Q: How long after I am hired will it take for me to have access to the portal? A: Dependent upon eligibility file frequency and when your district enters you into Benelogic. Q: What do I do if I have forgotten my username or password? A: On the main page of the portal at epcwellness.org, there is a link for Forgot Password? Under the Member Sign In section. This will help you recover your username, password of both. Q: I tried to recover my password but I never received an . How long should I wait for it? A: You should receive the recovery within a few minutes of your request. Be sure to check your spam folder. Also, be sure that you used the same address that you initially entered when signing up for your account. For example, if you signed up with your work but tried to recover the password with your home , you will not receive the . Q: What incentive dollars am I eligible for? A: If you are an employee enrolled in an EPC health plan through UnitedHealthCare or Anthem (as the primary subscriber), you are eligible for $35 for completion of your Personal Health Assessment and another $65 for the completion of a Wellness Workshop for a maximum of $100.
FREQUENTLY ASKED QUESTIONS Q: How long do I have to complete my activities? A: Employees have until August 31, 2015 to complete incentive activities. They have until September 30, 2015 to redeem their incentive dollars for a gift card. Q: How will incentive dollars be paid out? A: Incentive dollars will be paid in the form of gift cards. Q: What happens to my incentive dollars if gift card(s) are not ordered? A: Employees will have until September 30, 2015 to redeem incentive dollars for a gift card. If they do not redeem incentive dollars by that date, the dollars will be forfeited. Q: How long should it take to receive gift card(s) after they are ordered? A: If an employee selects a gift card that is available via it should be received within a few hours. If they select a gift card that has to be mailed, it can take up to two weeks to receive. Q: What happens if my gift card is lost, stolen or destroyed? A: EPC is not held responsible for lost, stolen or destroyed gift cards and is unable to replace them.
OUTREACH S AND PHONE CALLS All eligible employees are encouraged to participate in the wellness program. By participating you not only have the opportunity to learn more about your health, but also to earn incentive dollars To encourage participation employees may receive an outreach or phone call from an Wellness Program Engagement Specialist Employees who have completed the PHA but have not enrolled in a Wellness Workshop within 30 days of completing their PHA will receive an encouraging participation in a workshop Employees who have completed the PHA but have not started a Wellness Workshop within two weeks of receiving an outreach would receive a phone call from the Wellness Program Engagement Specialist to encourage participation All employees who complete the PHA and a Wellness Workshop will receive a follow up phone call from the Wellness Program Engagement Specialist to offer any further assistance or to answer any questions about the program
Individual health information will not be shared Overall participation information will be shared with EPC in order to help focus subsequent year programming needs Federal privacy standards are strictly followed CONFIDENTIALITY
