4 Called to Care TM Our Experience  VOA understands how to deliver service amongst affluent communities  Bailey’s Crossroads Community Shelter, Fairfax, VA  Bailey’s Drop-in Center, Fairfax, VA  Residential Program Center, Arlington, VA  Loudoun Drop-in Center, Cold Weather Shelter, Shelter and Transitional Housing 4

5 Called to Care TM Examples of VOA’s Service Platform  A homeless day drop-in center year around.  An emergency homeless shelter serving adult men and women each year.  A permanent supportive housing program providing services for chronically homeless men and women suffering from mental illness  Bailey’s and Loudon serve as a Hypothermia Shelters for homeless persons in Fairfax County during the winter months 5 Over 800 in Fairfax and 600 in Loudoun and 400 in Arlington

6 Called to Care TM Need in Howard County According To Point In Time Survey Conducted 1/27/2010:  221 Homeless Persons In Ho. Co.  106 Persons Were In Temporary Housing  88 Unsheltered  Last Year, Grassroots And Bridges To Housing Stability Turned Away 4200 (Duplicated) Requests For Shelter.  82 Unsheltered Homeless  Mostly Men  Persons Challenged With Mental Illness And Disabilities  Howard County Approved A 10-year Plan To End Homelessness 6

7 Called to Care TM Beechcrest Permanent Housing  33 to 50 Units Of Permanent Housing For The Homeless  Serving single homeless men and women  Multi-level Secured Building to be Constructed  Project Construction Likely To Begin By Early 2014  Construction Project Could Be Completed By Spring 2015  Current Residents In Community Will Receive Financial Support Through This Process That They Would Not Otherwise Have Had If The Site Was Redeveloped By A Private Entity 7

8 Called to Care TM Existing Grassroots Day Resource Center Beechcrest Day Resource Center (First Floor of New Facility) will replace and enhance the existing Grassroots Day Resource Center in Columbia, MD Which is Currently…  Open Monday, Wednesday and Saturday 10AM to 2PM  Serving men and women who are homeless  Holding Bi-weekly Town Hall meetings with Clients 8

9 Called to Care TM Proposed Staffing for Beechcrest Development  1 Program Director  3 Case Managers  1 Facilities Person  1 On-call Maintenance Manager  1 Receptionist/Administrative personnel  Extensive Community Involvement (e.g. Faith Based and Community Volunteers) who are Familiar with our Proposed Residents and Their Specific Needs  24 hours/7 days a week staffing pattern/Oversight 9

10 Called to Care TM Services – Day Center Comprehensive assessments and develop an Individual Service Plans (ISP) Mental health services Substance abuse treatment services Enrollment support for entitlement benefits Recreation Meals Medical care Spiritual & family connections Vocational or skills-building Transportation Social services Access to education Assistance with legal issues Clothing & personal hygiene Employment readiness & job placement Financial & healthcare benefit programs Life skills (cooking, housekeeping, laundry services, budgeting, etc.) Mental health care Nutritional & dietary needs Care Needs Assessment, Planning, and Coordination 10

11 Called to Care TM Hallmark of Service: Case Management  Individualized, personalized, comprehensive and intensive.  Use of Individual Service Plans:  Establishes the framework for services and individual efforts expected  Identifies the client’s goals and the short- and long-term objectives needed to reach these goals  Identifies the action steps required to achieve the objectives  Objectives defined in the plan are aimed at maintaining permanent housing and self-sufficiency 11
