3 First ACTS: Incident Report Creation First Student users who can create Incident Reports in First ACTS are called Writers. There are two types of First ACTS Writers. Locations can utilize one or both of the following types of Writers: Incident Reports are released directly to the schools Incident Reports require FS review before they are released to the schools FS Writer creates Report Report sent to School for review FS Writer creates Report Report reviewed at FS Report sent to School for review

4 First ACTS: Logging into First ACTS 1.Each First ACTS user should receive a “New Account Created” email message with their login credentials (system username & initial password) and link for their system 2.The link for the their system will open this Welcome page. In the Login box, enter the provided Username & Password to log into the system 3.New users will be prompted to set a new password

5 First ACTS: Creating an Incident Report 1.Student can be entered by Student ID or Name. If the student is found in the database, Grade & School will auto-fill. If student is not in the database, it will be added. 2.If student has any past incidents in the database, they will be displayed here and can be viewed by clicking incident number. 3.Incident information includes Date & Time, Route info. Infraction Type is from a pull-down list. Additional Infraction Types can be added by clicking the green “+” sign. Internal notes are visible to FS and School Reviewers, but not included on parent communications. 4.Files, such as photos or videos can be attached to the Incident Report 5.Once Incident Report is completed, click “Create Incident” to submit 1. 2. 3. 4. 5.
