Usually you solve a problem and you put in place an effective corrective action that prevents from re-occurring. So if you get a re-occurring issue, your corrective action was obviously not efficient. So let’s review the issue history and find out what was wrong, not sufficient etc. This can be supported by some simple questions instead of re- doing the whole 8D process. 3

Does it match with the today root cause? Maybe we had not considered other probable root causes or potential root causes.
