2 Program Evaluation Toolkit Pre-Award Preparations –Review Locator Form- Verify up-to-date locator information; add new information if needed (facebook, MySpace accounts) Multi-Agency Release of Information Forms –Identify all relevant agencies that will be providing services so client information can be obtained for follow-up & outcome analysis Data Collection Instruments –Review grant required surveys and identify any additional instruments that will be used for evaluation Client Intake Incentives (non-monetary) –Review Pricing »Flashlight (with evaluation cell number printed) »Follow-up Laminated Cards (necessary supplies) Construct Mock ‘Client Packets’ & ‘Client File Folders’ 2

3 Strategies to Build Rapport Client Enrollment into Program –Evaluators meet and personally enroll all clients –Evaluators review all intake forms with clients –Evaluators complete the locator form for the client which allows for client To locate phone numbers in their cell phones Creates smooth process if client reading ability is limited Creates rapport & relationship so follow-up is seamless Program Stakeholder Communications –Identify all program personnel (Court Director, Case Managers, Treatment Agencies, Probation/ Criminal Justice officers) that you will be working with during the program –Establish routine stakeholder meetings to review client indicators & provide evaluation feedback for stakeholders 3

4 Strategies for Follow-Up Interviews Tracking & Follow-Up Strategies – Send client postcards on major holidays (address check) –Utilize client population formats to schedule interviews Facebook, Text client, MySpace –One-month before interview eligibility window Begin contact to arrange interview; send reminders periodically; reminder call or text 24-hours before interview Utilize your stakeholders and their client knowledge –Check-in with Court Director- is client scheduled on upcoming docket; Case Management and Treatment Agencies communications on client status (have signed release from client) –Keep Detailed Log of all communication attempts & outcomes –Maintain In-House & GPRA database of key indicators such as Follow-Up Eligibility Window –Attend all Graduations, periodic court sessions, etc… activities to strengthen the client-evaluator relationship 4
