7 Birth Score WV State Code Article 22B. 16-22-B – 1-4 The purpose is to reduce the incidence of postneonatal mortality and disease by: Identifying newborns at greatest risk for death or developmental delay between one month and one year of age Linking these infants with physicians for close follow-up during the first year of life. Authorizes the division of health to establish and implement the Birth Score Program and requires hospitals, birthing facilities, attending physicians, and other persons attending a birth, to ensure a birth score is determined. 9 Newborn Hearing Screening WV State Code Articles 16-22A Article 22A requires the testing of newborn infants for hearing loss and requires that physicians or midwives attending a live birth assure that a test for hearing loss is performed Goal is to identify congenital hearing loss in children before three months of age and to assure enrollment in appropriate early intervention services before six months of age Mandated by WV State Law in 2000 Either Auditory Brainstem Response (ABR) or Otoacoustic Emissions (OAEs), following the equipment manufacturers guidelines, by personnel appropriately trained Screening must be performed in both ears and recorded in the medical record and on the Birth Score Developmental Risk Screen Infants who refer (abnormal screen) or are not screened are provided care coordination through the Right From The Start Program Website http://www.wvdhhr.org/nhs/http://www.wvdhhr.org/nhs/

10 Reports of Fetal Death WV State Code 16-5-21 Must be reported if 350 or more grams or weight unknown, or 20 or more completed weeks of gestation Must be reported within 5 days after occurrence Provider in attendance at delivery must complete and file report immediately after delivery

11 Compulsory Immunizations WV State Code 16-3-4 Required for all children entering school in WV Parent/legal guardian must be provided an official copy
