4 FORM 5 CHECK RIDE FORM 5 CHECK RIDE n Applicant must conduct prior preparation –Provide proof of proficiency flying with an instructor/or self within 90 days as per FAR 61.57(a)(1). (CAPR 60-1, Attachment 5) –Complete CAPR 60-1 Written Exam (Powered or Glider). –Obtain necessary forms (including personal copy of CAPR 60-1). –Complete aircraft questionnaire for applicable aircraft as outlined in table 3-1 or for applicable glider. –BE FAMILIAR WITH CAPR 60-1 AND FAR’s. n Be in proper uniform & have flight release.

5 FORM 5 CHECK RIDE FORM 5 CHECK RIDE n Check Pilot will: –Review all paperwork (certificates, medical, etc.) –Ensure it is filled in by applicant in appropriate area. (Applicant should paperclip/post-it note needed info) –Check pilot must fill in verification area in back of form. n Conduct ground evaluation –Review CAPR 60-1, FAR’s, Weather, Planning. –AROW (Airworthiness, Registration, Operating Limitation, Weight and Balance). –Develop a mock flight route (performance, weight, etc). n Conduct flight evaluation –Conduct to applicable PTS. –Instrument portion should be conducted by CFII. –THIS IS AN EVALUATION NOT INSTRUCTION

6 FORM 5 CHECK RIDE FORM 5 CHECK RIDE n Post Flight –Review performance based on applicable PTS. –Complete all paperwork. n Applicant is PIC during flight unless agreed to in advance (i.e. Safety Pilot). Applicant should receive instruction prior to Form 5/5G Check Ride to be FAA Current. Form 5 Check Rides should not be performed during training/actual SAR/DR/CN mission. n Unsatisfactory check rides must be reported to the Wing DOV ASAP.

7 FORM 91 CHECK RIDE FORM 91 CHECK RIDE n Applicant must conduct prior preparation –Be current and qualified IAW CAPR 60-1 and FAR’s. –Be familiar with CAPR 60-1, 3, 4 (pt 1 & 2) & 5. –Be currently qualified in aircraft used for the mission. –Obtain necessary forms. –BE FAMILIAR WITH CAPR 60-1, AND FARs. n Check Pilot will: –Ensure compliance with uniform, id card, flight release. –Review all paperwork (certificates, medical, etc.) –Ensure it is filled in by applicant in appropriate areas. –Verify current Form 5 Check Ride in aircraft to be used.

8 FORM 91 CHECK RIDE FORM 91 CHECK RIDE n Form 91 Mission Check Ride Procedures n Oral Review –Mission procedures, mission planning, search patterns, etc. n In-Flight Portion –Applicant is pilot in command. –Mission check pilot shall observe all flight maneuvers. –A practice ELT MUST be used for electronic search portion. –Emergency procedures should be performed verbally by non-CFI mission check pilots. Mission flight maneuvers are to be conducted with safety as priority. Maneuvers will be conducted at 1,500 AGL or above.

9 FORM 91 CHECK RIDE FORM 91 CHECK RIDE n Post Flight: Review and Documentation –Review performance and discuss any comments. –Complete the CAPF 91. –Return completed paperwork and forward one copy to Wing HQ for DOV. n This is an evaluation not an instructional session. Limited instruction may be provided for minor items. n Unsatisfactory check rides must be reported to Wing DOV ASAP.

10 PILOT AUTHORIZATIONS n All pilot authorizations must be designated on Wing orders signed by the current Wing Commander or his/her designee. n Cadet Orientation Pilot –Be qualified as per CAPR 60-1 and FAR’s. –Be very familiar with CAPP 52-7. –Recommend completion of NHQ online Cadet Orientation written test. –Have a high level of proficiency and fly on a regular basis –Be approved by Check Pilot during Form 5/5G.

11 PILOT AUTHORIZATIONS n Instructor Pilot –Be qualified as per CAPR 60-1 and FAR’s. –Complete Instructor specific Form 5/5G with a Chief Check Pilot and be endorsed as being qualified for instructor pilot. n Check Pilot and/or Mission Check Pilot –Be qualified as per CAPR 60-1 and FAR’s. –Must attend and successfully complete the NCPSC prior to being designated. –Must have flown at least 25 missions prior to applying for Mission Check Pilot. –Must receive a Form 5/5G/91 for qualification by a Chief Check Pilot or Mission Check Pilot designated by Wing DOV.

12 FLIGHT RELEASE n All CAP flights must be properly flight released. n The PIC must obtain the flight release and has to speak directly to the FRO. n The flight must be IAW CAPR 60-1, para 4-2 or 4-3. n Non-Corporate (member owned, etc.) aircraft used for CAP flights must be flight released just like a Corporate aircraft. n If anything about the flight changes (passengers, destination, purpose, etc.), the PIC must obtain an updated flight release prior to takeoff. n Total flight time (Hobbs) must be reported to the FRO at the end of the flight. n Failure to obtain a proper flight release may result in the pilot being grounded.

13 FLWG TRENDS FLWG TRENDS n Form 5/91 Checkrides –Members with 8-10 hours (or less) of flying within the last 12 calendar months. –Lack of proficiency flying within 6 months of check ride. –Members performing only minimum FAA requirements for currency. –25 to 35% of pilots do not have copy of CAPR 60-1. –25 to 30% of candidates arriving for check ride not prepared. –25 to 40% check ride failures. –40 to 60% demonstrate marginal performance.

14 RESULTS RESULTS –Two airspace/runway incursions over the last 24 calendar months. –Two aircraft incidents in the last 12 calendar months with damage totaling over $75,000. –Aircraft flown with expired Temporary registration. –One falsified Form 5 in the last 12 calendar months. –Two incidents where member was not in uniform while flying.

15 RESULTS RESULTS –One CAP aircraft intercepted by USAF F-15 fighters at a cost to the Air Force of over $15,000. –At least three separate occasions where Cadets were flown without a proper Flight Release being obtained. –Several Cadet flights made by pilots not on Wing orders. –One aircraft was flown for 120 days after the transponder was due for inspection. –One aircraft flown 11 hours past 100 hour inspection

16 SOLUTION SOLUTION –PILOT PROFICIENCY !! –KNOW AND FOLLOW THE REGS - BOTH FAA AND CAP –PRACTICE FLIGHT MANEUVERS ON A REGULAR BASIS (CAPR 60-1, attachment 7) –RECURRENT FLIGHT TRAINING WITH CFI –FAA WINGS PROGRAM –Remember, Pilot Proficiency is NOT optional in CAP... it’s MANDATORY !

17 FRO TRENDS n Problems with Form 99’s –Using obsolete forms –Not filled out properly with identifying data: n Unit, Wing, Region and Dates missing –Not legible –Signatures missing –Flight times missing –Wrong mission symbols being used –Form 99’s not being sent to LO by the 5th of each month. –Original Form 99’s not being sent to Wing DO by the 5th of each month.
