1. Exciting news for International Students! On August 6, 2009, F-1 students were automatically enrolled in the Aetna Health Insurance Company. We are excited about the new changes Aetna will bring and how much easier it will be for you! Here are a few changes we think you will like: Students will no longer have to submit claim forms! An optional dental plan is offered on the Student Insure website. www.studentinsure.com/luwww.studentinsure.com/lu Once you are enrolled with Aetna, you may choose to enroll separately online for an additional $262 per year. New and Returning International Students You are now able to go to www.studentinsure.com/lu to obtain an Aetna Health Insurance Enrollment Card. www.studentinsure.com/lu After following the link, please follow the steps in this PowerPoint. You must do this immediately to verify that you are covered with student health insurance. You should carry your insurance card with you at all times.

3. Follow these steps to get your Insurance Card: 1. Go to www.studentinsure.com/lu www.studentinsure.com/lu 2. Click on “Member Services” 3. Click on “Get ID Card” 4. Enter first and last name and date of birth 5. Click on the current year policy 6. Click on “Print ID Card”

4. To Activate your insurance follow these steps: Once you have printed your card, you need to activate it. Go back to member services and click on the “update member info” tab. Enter your Insurance ID number. Your password will be your last name. Update your address and phone number. You are finished! Your insurance card is now activated!

5. How to use your Prescription Drug Benefit Take your written prescription to any pharmacy and pay for the prescriptions at the time of purchase. Go to www.studentinsure.com/lu to print a prescription drug claim form and complete the top portion.www.studentinsure.com/lu Send the claim form along with a copy of the receipt that was stapled to the bag the pharmacy gave you. 1 2 3

6. How to use your Prescription Drug Benefit Your prescription claims are subject to the same $100 deductable per year as your medical claims are. You will be reimbursed for 100% of the cost of your prescriptions after you first meet you deductible. Once you send your claim to Aetna, it will take 3-4 weeks to process. You may call Aetna by using the number listed on your insurance card for updates about your claim. 4 5 6

7. Other Health Insurance Items to Remember: We strongly recommend that you read the new 2009/2010 Aetna Health Insurance Policy to familiarize yourself with the medical treatments that are covered. A copy of the policy can be found at the International Student webpage. www.Liberty.edu/international www.Liberty.edu/international You do not need to submit a claim form when using Aetna Health Insurance. Aetna may send you an additional information form for you to fill out and send back to them if they have preexisting condition questions about your claim.

8. More Health Insurance Facts Your policy has a $100 deductible per year. This means that you will be required to pay up to the first $100 of medical costs after the first time you visit a doctor.
