5 :Introduction Multidisciplinary care when professionals from a range of disciplines work together to deliver comprehensive care that addresses as many of the patient's needs as possible. This can be delivered by a range of professionals functioning as a team under one organizational umbrella or by professionals from a range of organizations, including private practice, brought together as a unique team.

6 Definition of team: a group of people with complementary skills who are committed to a common purpose, performance goals, and approach, for which they hold themselves mutually accountable.” Multidisciplinary team: in the context of health care, means that health care providers from different professions work together to collaboratively provide diagnoses, assessments and treatment, within their scope of practice and areas of competence.

7 The definition of Multidisciplinary care :
is a collaborative approach to treatment planning and ongoing care throughout the treatment pathway. primary purposes : Help team members resolve difficult cases, Promote coordination between agencies; provide a "checks and balances" mechanism to ensure that the interests and rights of all concerned parties are addressed. Identify service gaps and breakdowns in coordination or communication between agencies or individuals. Enhance the professional skills and knowledge of individual team members by providing a forum for learning more about the strategies, resources, and approaches used by various disciplines.

8 The definition of multidisciplinary field:
An interdisciplinary field: is a field of study that crosses traditional boundaries between academic disciplines or schools of thought, as new needs and professions have emerged. Originally the term interdisciplinary is applied within education and training pedagogies to describe studies that use methods and insights of several established disciplines or traditional fields of study.

9 The involvement of multidisciplinary field:
Researchers, students, and teachers in the goals of connecting and integrating several academic schools of thought, professions, or technologies along with their specific perspectives in the pursuit of a common task.

10 Composition of multidisciplinary team:
Group composed of members with: varied but complimentary experience, qualifications, and skills that contribute to the achievement of the organization's specific objectives. While their primary purpose is typically to help team members resolve difficult cases, teams may fulfill a variety of additional functions.

11 The definition of multidisciplinary teamwork:
Multidisciplinary teamwork: The timely and cooperative application of all appropriate disciplines in an open-communication, shared-information environment to effect people functioning as a team to achieve optimum solutions (people, product, and process) satisfy customer needs, objectives, and requirements.

They have competencies in the areas of: Assessment Treatment and care management Collaborative working Management and administration Interpersonal skills.

14 1. Assessment This is the first step in providing comprehensive health care for individuals. Skills in this area include: familiarity with the key skills required to initiate and maintain contact with service users, and ability to use these skills to engage with service users and careers, ability to conduct a collaborative needs-based assessment; ability to develop a treatment and care plan based on a thorough and comprehensive assessment of the service user, family and social system;

Assessment Assessment of users’ needs and requirements of housing, occupation and income; Ability to apply knowledge and skill in risk assessment and the management of violence and aggression. 2. Treatment and Care Management knowledge and skill in effective inter-personal communication; knowledge and skill in creating therapeutic co-operation and developing an alliance with the service user; awareness of user perspectives on the provision of treatment and continuing care; understanding of the philosophy and background behind recovery principles

16 3. Collaborative Working
Ability to work effectively as a member of a multi-disciplinary health team through clarity about the role and purpose of the team and its individual members. Ability to work in partnership with service users, careers and social networks; Understanding of sources of conflict and development of basic teamwork skills including negotiation and conflict resolution; Comprehension of the need for and willingness to participate effectively in multi-disciplinary team supervision; Knowledge of what leadership is and how it differs from management, and how good leadership can improve outcome for individual service users.

Awareness of the role and contribution of non-specialist and support staff, the ability to supervise and provide support to those staff; Ability to maintain clear and accurate multidisciplinary notes; understanding of service planning, audit and evaluation; understanding and appreciation of clinical governance (Clinical governance is a process which aims to improve the standards of clinical care through continuous audit and evaluation of outcomes awareness of the unique competencies/skills of other team members

18 Interpersonal skills Respect for service users and cares as individuals and understanding of the rights of users and cares; Respect for and understanding of, the different training, skills and perspective of other team members; An understanding of multidisciplinary team working and commitment to health treatment and care; cultural awareness - an awareness relevant to their client groups , professional, respect for diversity cultural, models of working etc.)

19 The definition of multidisciplinary approach:
Multidisciplinary approach: includes the drawing appropriately from multiple disciplines (such as sociology, geography, planning, engineering, architecture and design, economics, public health, cybernetics, statistics, systems thinking etc) to define and apply new ways of understanding complex situations. Most recently this method of working has been adopted in the specialist field of computer security.

20 The consist of multidisciplinary team:
General practitioners. Practice nurses. Community health nurses. Allied health professionals (may be a mix of state funded community health and private professionals) such as physiotherapists, occupational therapists, dietitians, psychologists, social workers, podiatrists and Aboriginal Health Workers. Health educators - such as diabetes educators.

21 The benefits of multidisciplinary team:
Improved health outcomes. Enhanced satisfaction for clients. More efficient use of resources. Enhanced job satisfaction for team members. staff acquire new skills, participate in decision making and take on more delivering services that are planned and co-ordinated delivering services that are cost effective . enhancing information sharing and reorganization of work practices

22 The role consideration in the multidisciplinary team:
Respect and trust between team members. The best use of the skill mix within the team. What clinical governance structures need to be in place. How communication and interaction will occur between team members.

23 The ideal participant of multidisciplinary team:
Is a part of the team and identifies himself/herself with the team’s goals. Is empowered and entrusted with the task. Has the support of his/her manager. Is experienced and knowledgeable in his/her field. Represents colleagues. Has the time to participate. Wants to contribute to the success.

24 The multidisciplinary teamwork depends on:
Putting together, establishing and sustaining a team. Doing all important team-based work in facilitated workshops. Visualizing and documenting the team’s efforts throughout the process.

25 The definition of multidisciplinary team meeting:
A Multidisciplinary Team Meeting: is a meeting of the group of professionals from one or more clinical disciplines who together make decisions regarding recommended treatment of individual PATIENTS.  Multidisciplinary Teams may specialize in certain conditions, such as Cancer.  Clinical decisions are made based on reviews of clinical documentation such as case notes, test results, diagnostic imaging etc.

26 The summary: Multidisciplinary team: A group of people with different kinds of training and experience working together, usually on an ongoing basis. Professionals often use the word 'discipline' to mean a 'field of study,' such as medicine, social work, or education. They also enhance the professional skills and knowledge of individual team members by providing a forum for learning more about the strategies, resources, and approaches used by various disciplines.
