4 Page 4 of 13 Login to ETS 1. Login to ETS with your user name and password 3. Select Overhole/ Coring 2. Expand Crown Mineral Activity

5 Page 5 of 13 a.Admin- this screen contains Contact information and Technical Contact if needed. b.Well Details – this screen displays the Well Type, Rights, Substance, Meterage, ERCB order, and Well Information for this CMA type. b.Geological Data – this screen allows you to submit geological information for this form. Overhole/Coring Tabs

6 Page 6 of 13. Complete Admin Tab 2. Enter Contact Information 1.Enter Comments, Select Company Name Enter Technical Contact Information if available 3. Click Save

7 Page 7 of 13 1.Check the appropriate Well Type 2. Click on Qualifier and Zone dropdown to choose an option 3.Check the appropriate Substance 4. Enter the Meterage Complete Wells Details Tab – Well Type, Rights, Substance, and Meterage Meterage is required if greater than 15 meters.

8 Page 8 of 13 Complete Wells Details Tab – ERCB Order and Wells Alternatively, you can browse and upload a file in.CSV (comma separated) file format containing multiple well IDs. 4. Click Add Well(s) 2. Enter Unique Well Identifier 5. Click on Save 3. Click on the find button ( ) to search for a Crown agreement 1. Browse and select a PDF file An AER/ERCB order is required. Browse and select from your directory to attach this document. After saving, proceed with the rest of the Well Details tab.

9 Page 9 of 13 Complete Wells Details Tab – Well ID – File Upload Format The CSV file must follow this format: The file contains 2 columns and must be uploaded comma “,” separated. First Column is Well ID containing 20 characters including the “/” and “-”: Well ID format is 99/99-99-999-99X9/99 Second Column is Agreement Number containing 10 characters Agreement Number format is 9999999999 Sample file to upload: 02/14-29-053-06W4/00 0499999999 03/06-29-053-06W4/000599999999 03/14-29-053-06W4/000699999999

10 Page 10 of 13 Complete Geological Data Tab 2. Check the Appropriate box 3. Click Save 1.Browse and Select a PDF document A Geological Discussion document is required. Browse and select from your directory to attach the document and save.

11 Page 11 of 13 Submit Overhole/Coring Crown Application Click on Submit Click on the View Report link to review the details of this CMA Application.

12 Page 12 of 13 Submit Overhole/Coring Crown Application - Indemnification Click on Yes Click on OK Refer to the Work In Progress module for more information on the status of a submitted Application Request.
