An Authorization Request may be required if the applicant is not the designated representative of the Crown agreement or the well licensee. 4 Page 4 of 14 Login to ETS 1. Login to ETS with your user name and password 2. Expand Crown Mineral Activity 3. Select Well Re-entry

5 Page 5 of 14 Well Re-Entry Tabs a.Admin - this tab contains Contact information and Technical Contact, if needed. b.Well Details - this tab displays the Well ID, Agreement, Formations, Terminating Location, and other Well Information for this CMA type.

6 Page 6 of 14 Complete Admin Tab Enter Technical Contact Information if available. 1.Enter Comments, Select Company Name 2. Enter Contact Information 3. Click Save

7 Page 7 of 14 2. Browse and select a PDF File. 1. Click on check box if required 3. Click on Save A Letter of Explanation is required when an existing approval is due to expire or has expired. Browse and select from your directory to attach this document. After saving, proceed with the rest of the Well Details tab. Complete Well Details Tab – Attach Letter of Explanation

8 Page 8 of 14 Complete Well Details Tab – Enter Well ID 4. Enter Unique Well Identifier

9 Page 9 of 14 2. Enter Crown Agreement Number 3. Click on Search 4. Select the checkbox 5. Click OK Ensure to SAVE after completing a search for a Crown agreement. 1. Click on the find button ( ) to search for a Crown agreement Complete Well Details Tab – Add Agreement Number

10 Page 10 of 14 1. Enter Previous Approval Number if applicable 2. At least one Purpose and one Substance must be selected. An AER Order is mandatory if the purpose of Repair/Re-abandoned is selected. Browse and select a PDF file for the AER Order. Complete Well Details Tab – Well Details

11 Page 11 of 14 1. Click on the Formation dropdown to select an option 2. Click Add 4. Click on Save 3. Select and Add a Terminating Formation from the dropdown Complete Well Details Tab - Formations The Terminating Formation must be selected if different from the (In the) Formation.

12 Page 12 of 14 Click on Submit Submit Well Re-Entry Application Click on the View Report link to review the details of this CMA Application.

13 Page 13 of 14 Click on Yes Click on OK Submit Well Re-Entry Application - Indemnification Refer to the Work In Progress module for more information on the status of a submitted Application Request.
