4 Page 4 of 13 Login to ETS 1. Login to ETS with your user name and password 2. Expand Crown Mineral Activity 3. Select Undisposed Crown

5 Page 5 of 13 a.Admin- this screen contains Contact information and Technical Contact if needed b.Well Details – this screen displays the Well Type, Formations, Substance and Well Information for this CMA type. b.Geological Data – this screen allows you to submit geological information for this form. Undisposed Crown Screen Tabs

6 Page 6 of 13 Complete Admin Tab 1.Enter Comments, Select Company Name 2. Enter Contact Information 3. Click Save Enter Technical Contact Information if available

7 Page 7 of 13 1.Check the appropriate Well Type 2. Click on Formation dropdown to choose an option 3. Click Add Complete Wells Details Tab – Well Type and Formation Multiple formations can be selected.

8 Page 8 of Click on Save 1.Check any appropriate Substance(s) 2. Enter Unique Well Identifier 3. Click Add Well(s) Alternatively, you can browse and upload a file in.CSV (comma separated) file format containing multiple well IDs. Complete Well Details Tab – Substance and Wells

9 Page 9 of 13 Complete Well Details Tab – Well ID – File Upload Format The CSV file must follow this format: The file contains only one column and must be uploaded comma “,” separated. The Column is Well ID containing 20 characters including the “/” and “-”: Well ID format is 99/ X9/99 Sample file to upload: 02/ W4/00 03/ W4/00 03/ W4/00

10 Page 10 of Click Save 2. Check the Appropriate box 1.Browse and upload document Complete Geological Data Tab A Geological Discussion document is required. Browse and select from your directory to attach the document and save.

11 Page 11 of 13 Click on Submit Submit Undisposed Crown Application Click on the View Report link to review the details of this CMA Application.

12 Page 12 of 13 Click on Yes Click on OK Submit Undisposed Crown Application - Indemnification Refer to the Work In Progress module for more information on the status of a submitted Application Request.
