1. W HAT IS THIS RESEARCH ABOUT ? HABSelect is looking if there are better ways of choosing the 'best' sperm for injection in ICSI than the current ones HABSelect is going to use special plates coated with the substance called 'hyaluronan'. It will act as glue but only for the ‘best’ sperm which has matching receptors. HABSelect is trying to find out if this form of sperm selection can improve the pregnancy outcomes HABSelect will also try to find out if sperm selected by hyaluronan is different than the one without i.e. have less DNA damage and how that could be helping in achieving successful pregnancy – that is why we will collect the sperm left-over after your treatment but use for research purpose only

2. W ILL OUR TAKING PART BE KEPT CONFIDENTIAL ? Yes, all the information collected will be kept with Data Protection Act and your clinic Code of practice Any information entered into secure, electronic study database will be stripped of your identifiable details, using study number instead Any sperm samples collected for study purposes will use the same study number instead personal details I S THIS RESEARCH SAFE ? Hyaluronan selection equipment has been extensively tested and obtained all necessary approvals for clinical use in European Union

3. H OW MUCH OF MY TIME THE STUDY WILL TAKE ? During Patient Evening or individual appointment with the clinician or nurse you will be given a leaflet on the study - you can take as much time as you wish to make up your mind if you want to take part in the study HABSelect assessment are based on the routine ones (hormonal tests etc) which are performed for your treatment purposes HABSelect intervention takes place during your ICSI treatment and applies only to the process of sperm selection There are no additional procedures after that and only the data on the pregnancy outcome is collected as per routine standard of your Fertility Clinic
