1. This DBA application comprises of 3 switchboards, 19 forms one of which has a 3 tab format and 6 reports. In this presentation, only some of the more complex forms will be shown as well as some reports.

4. After customer login, selection of the buttons below will open the correct data input form with the associated customer’s name..

5. This form consist of a main form (customer’s name) and a sub form for the job data input. Once data is selected in the dropdown list, the fields at the bottom of the form will be populated with the same information.

6. After the “COUNT” field is inputted, the TASK SUMMARY data populates an invoice for the associated customer.

7. Price is found in the QUOTES report indicated by the “QUOTES” button. When the PRICE PER column is filled, the COST OF ITEM for each row is auto calculated and the final cost is shown in the TOTAL field.

8. This is a 3 tabs sub form for the 3 different job order types a customer can have. Selecting the JOB# and the Customer’s Job Invoice Form button opens the invoice form [previously shown] for that particular customer and job order.

9. Selecting the “VIEW COPY REPORT” in the previous screen will open all customers with Copy Orders. This report is also available for the Imaging and LBG job orders.

10. While this form does not have the advance features of auto populated fields, clicking the BATCH-BOX-FILE FORM button opens a linked sub form that has 2 nested sub form -all associated to one job order, #16 in this case.

11. This linked sub form opens with the associated job order. It will keep track of files done in the volume of boxes within the numerous batches for one particular job order with a given customer. This type of form can only be realized by means of solid DBA relationship modeling as well as applying DBA concepts and/or rules.
