2 Outcome: Recovery thru Support in Housing Approach Choose … Maintain security of tenure Motivated and make informed decisions … In recovery Successful tenant Indicators Measures Tenants who apply are housed (% applied are accepted) Tenants with complex needs are housed (1-5%) (% applied are accepted) Tenants choose housing (% tenants who accept unit) Tenants living in deep core poverty have housing they can afford (RGI, average tenant income) Tenants have a support plan (OCAN, welcome checklist) Tenants learn about being a tenant (% tenants who get handbook, orientation, welcome surveys) Tenants receive the support they need (from Mainstay & partner agency) (via welcome surveys) Tenants maintain housing (number of tenants who remain housed against benchmark) Tenants choose to be good neighbours (% where no N5, N6, N7) Tenants access support & counselling (contact stats) Tenants know how to work with landlord (# cleaning related work orders, outcome annual inspections, % after hour calls that are reasonable) Tenants know how to pay rent (# of tenants who choose to use payment plan rather than go into arrears, % of successful repayment plans) Tenants who use Mainstay Opportunity Fund (# who accept, use fund) Tenants become WRAP graduates (# who participate in intro, level 1 & level 2) Tenants can articulate their understanding of ‘recovery’ (through WRAP) Tenants participate in a support plan (via OCAN, Streets to homes @ rent supplement records) Tenant manage hoarding (% safety plans in place / adhered to) Tenants maintain long term security of tenure (number of tenants who maintain housing against long term bench mark) Decrease in (premature) deaths for people with mental health & complex needs (system number, research) Decrease in access to (unnecessary) emergency/ primary care/hospital for People with mental health & complex needs (system number, Streets to Homes) Tenants who exercise choice in housing moves (% from move out check list) Promoting Housing Choice & Recovery for tenants with mental health and complex needs
