3 Step #1: Referral Form Who can Refer Students? Teacher Parent Self Peer When to Identify? We recommend as early as 3 rd Grade

4 Step #1: Referral Form Continued Parental Permission to Screen/Assess If a parent is the one referring the student, complete permission section at the bottom of the referral form Otherwise a separate permission form is needed District Level Permission Form Check for an existing required permission form from your district

5 Step #1: Referral Form Continued Music Check List on Referral Form (must be able to check at least one area) ____ Matches pitches accurately ____ Is able to duplicate complex rhythms correctly ____ Demonstrates unusual ability on an instrument including voice ____ Has a high degree of aural memory/musical memory ____ Displays compulsive musical pursuit

6 Step #2: Student Profile Sheet Background Knowledge of Student and Experience Private lessons Coursework related to music Opportunities to critique music Music based extra-curricular activities or clubs Additional relevant information

7 Step #3: Behavioral Check List Renzulli’s Behavioral Check List of Musical Characteristics 1. The student shows a sustained interest in music ---seeks out opportunities to hear and create music. 2. The student perceives fine differences in musical tone (pitch, loudness, timbre, duration). 3. The student easily remembers melodies and can reproduce them accurately. 4. The student easily participates in musical activities.

8 Step #3: Behavioral Check List Continued 5. The student plays a musical instrument or sings, or indicates a strong desire to do so. 6. The student is sensitive to the rhythm of music (beat); responds to changes in the tempo of music through body movements. 7. The student is aware of and can identify a variety of sounds heard at a given moment---is sensitive to “background” noises, to chords that accompany a melody, to the different sounds of singers or instrumentalists in a performance.
