3 Page 1 of the Application Guidance Notes are located in the gray boxes to the right of the application form. They run the length of the application and include detailed explanations of the questions, instructions on how to fill out the form and useful links to documents and contacts Begin by selecting the Principal Investigator. If you have previously held a PI role, this field may automatically be populated with your name.

4 Filter by the last name and include a “%” in front of the keyword to maximize results Select the name of the Principal Investigator Click “Ok” to add your selection

5 Please take notice that some questions are marked with a red asterisk and indicates that the question is a required field. If you leave these questions unanswered, you will not be allowed to proceed to the next page of the application Make it a habit to hit “Save” before you “Continue” to the next page

6 Page 2 of the Application Whenever you encounter questions that require you to input a date, please use the calendar icon to select your dates as it will ensure the proper formatting of your entry.

8 You will encounter questions where you must click “Add” to select an item from an established list Page 4 of the Application

9 Option 1: Search for the first letter of the institution Option 2: Simply leave the field blank and click “Find” for the complete list v for “VCHRI” p For “Providence Health Care”Etc…

10 Select one or more Institutions and the corresponding sites where your research will take place Click “Ok” to add your selection

12 Page 9 of the Application Attach documents into the body of your application by clicking “Add”

13 RISe Tutorial Protocol Name your document Include the document version number 1 Select a version date February 12, 2007 Browse your computer for the document you want to attach Click “Ok” to add your selection

15 Last Page of the Application “Save & Close” out of the application – You will automatically be taken to the Study Homepage

16 The Study Homepage A newly generated Study ID NumberYour Study Nickname To track the status of your application through the approval process, refer to the “Current State” box Activities that you may perform As the state of your application is “Pre Submission”, your application is still open for any edits you may want to make – click on the “Edit Application” button to go back into the application.

17 Use the Jump to: tool to navigate the pages of the application Click “Save” as you navigate between pages Click “Exit” to return to the Study Homepage

18 Click to submit your new application for review

19 Select the appropriate department for the review and approval of your application Click “Ok” to send your application

20 Notice the state of your application is now “Department Review” and your application is awaiting approval from the Head of the Department of Medicine All your activities and comments can be viewed under the Correspondence tab
