Your annual chance to review your benefit elections November 1 – November 30 of each year Most employee changes made on-line at: http://hrss.umn.edu Same sex domestic partner applications done by paper – not available online at this time Additional information on Employee Benefits website at: http://www.umn.edu/ohr/benefits/

Elect to participate in the Health Care and /or Dependent Care Flexible Spending Account New election required each year Elect to participate in Voluntary Long Term Disability coverage for Civil Service and Represented employees, or change your LTD benefit amount

6 Human Resources Office of What if I don’t want to change anything? You don’t need to go on-line and change your elections unless you’re an FSA participant and want to continue for 2007 if you only want to change clinics, call your health plan directly 2006 is a voluntary open enrollment – no changes required

You will receive a new card if you enroll in a new plan These plans will issue new cards: All Medica Plans HealthPartners U Classic Plus Medical Plan Delta Dental Plans These plans will not issue new cards RxAmerica HealthPartners Dental Plan

8 Human Resources Office of When are the Benefits Fairs? UMD – November 2 - Kirby Student Center St. Paul – November 8 – St. Paul Student Center Minneapolis – November 9 – Coffman Memorial Union – Great Hall All Fairs run from 10 a.m. to 3 p.m.

9 Human Resources Office of Where can I get a flu shot? Free Flu Shots at all Benefit Fairs Other Flu Shot clinics at all campuses No copay for flu shots at MinuteClinic No copay for flu shots at your own medical clinic

11 Human Resources Office of Are the same Medical Plans available? Yes, all options remain the same as in 2006 The base plan choices are: Medica Elect / Essential for Twin Cities and Duluth Medica Choice Regional for Greater Minnesota

All other plans also remain the same: U Classic Plus by HealthPartners Insights by Medica (tiered network) Medica Choice National (open network) Medica Direct Health Reimbursement Account (HRA) Medica Direct Health Savings Account (HSA)

13 Human Resources Office of Are there changes within any of the Medical Plans? Most plans remain the same Changes are: New Travel Benefit for Base Plans and Insights by Medica Copay Increases for Insights by Medica Medica Direct HRA / HSA Program Improvements

New Travel Benefit added to: Medica Elect and Essential Medica Choice Regional Insights by Medica U Classic Plus by HealthPartners Note: National coverage already available in Medica Choice National and Medica Direct HRA / HSA Plans

Permanent residence must be in MN or surrounding states Must use network providers: Contact your plan, either online or by phone, to determine which providers you may use. Medica’s network excludes certain providers

25 Human Resources Office of Does the UPlan Pharmacy Program have any changes for 2007? No, RxAmerica will continue as the pharmacy benefit manager for all UPlan Medical options in January, 2007. Rx copays will remain the same for 2007 for all plans except the Medica HRA/HSA’s Generic Plus $10 Formulary Brand $20 Non- Formulary $35

26 Human Resources Office of What else should I know about the UPlan Pharmacy Program? If you or your doctor request that you take a brand drug when a chemically equivalent generic is available, you pay the Generic Plus copay + the difference between the cost of the generic and brand drugs, unless you have a prior authorization in place.

41 Human Resources Office of Are there any changes in Voluntary Disability coverage? Effective 1/1/2007 Voluntary Short and Long Term Disability coverage will move: Standard Insurance Company  Well-respected Disability Insurance carrier  Located in Portland, Oregon  Founded in 1906

Benefits remain the same Employees with coverage now will automatically transfer to Standard Insurance Company Employees out on disability will remain with the current carrier All STD and LTD rates will be reduced!

43 Human Resources Office of Can I enroll in Long Term Disability during Open Enrollment? Civil Service / Represented Employees may enroll in Long Term Disability coverage during open enrollment Pre-existing Condition Limit: Disabilities related to medical issues occurring during 2006 will not be covered for 24 months

44 Human Resources Office of Can I enroll in Short Term Disability during Open Enrollment? You can enroll or increase your Short Term Disability coverage at any time. New STD enrollments or increases in coverage will require proof of good health If interested, contact Employee Benefits for a paper application, and instructions for providing health information Changes effective 1/1/07 or later

45 Human Resources Office of Are there changes to the Faculty Income Protection Program? FIP is renamed the Academic Disability Program. Changes from Minnesota Life to The Standard effective 1/1/2007 All coverage automatically transfers Additional information will be sent to covered Faculty and P&A members

46 Human Resources Office of Will the claims process change? New telephonic claims filing process begins 1/1/2007 No forms to complete for disability claims Medical information goes directly to The Standard, not through your department FMLA claims are still filed with your department

48 Human Resources Office of When can I change my Life Insurance? Life Insurance Additional Life Insurance can be increased or decreased at any time during the year Contact Employee Benefits for a paper application Proof of good health will be required for any increases

Long Term Care Insurance Coverage will change from CNA to John Hancock, effective 4/1/2007 New program will include:  Improved benefits  Lower rates for many participants  Expanded eligibility  Guaranteed issue coverage for all active employees Detailed information will be provided beginning in January

51 Human Resources Office of What’s new for UPlan Wellness? 10,000 Steps Available to all U of M employees  + Spouses/SSDPs and Early Retirees on UPlan  Enroll by 12/31/2006  Complete in 2006 or 2007 Free pedometer, on-line step tracking Motivational messages Employees enrolled in the UPlan who have not received a second $65 reward can earn their reward for completing 10,000 Steps
