4 Preferred zones - pilot To guarantee equity in the allocation of a zone where placements for each student will take place for the duration of their course: – students are asked to complete a student profile form that includes: choices in order or preference for geographical zones *personal information to facilitate allocation * Information supplied will be used by SHU in accordance with the Data Protection Act

5 Personal Profile At the beginning of the course you will be asked to fill personal profile form – deadline is Thursday October 5th, 5 pm – included is your zone selection You will be notified via Blackboard your allocated zone for the duration of your course the weeks of October 9th-16th During your course, please update additional information on-line via My STUDENT Record – example: wards that may cause conflict of interest

7 Does this mean I will be rotated to all three areas of my zone every year? Your zone is where your placements can occur, but the actual allocation will be dependent on: – suitability of learning experience – available placements – a random number assigned to each student in each placement block that will determine the order you will be placed - equity – your personal profile items – your zone

8 Other student profile information: On-line student profile – encourages student responsibility: – Caring needs – Transportation – GP – Not appropriate placements (family or friends in wards) – Any other – Preferential Geographical zone

9 Other on-line information Your placements – you will be able to see in which ward/placement you have been allocated On-line attendance form: to be filled at the end of each placement (deadline is one week after the end of the placement), signed by tutor and handed in to the Placement Team – attendance deadlines will be posted on First Class/Blackboard FAQs – on Blackboard and on web-link (available shortly)

10 Important information! Students are responsible for updating their student profile Requests by students or academic staff for swaps/alternative placements will not be considered Students cannot refuse a placement allocated to them It is the students' responsibility to arrange shifts/rotas with mentor and to make necessary arrangements for travelling and accommodation to/from placements Mentors are under no obligation to accommodate your preferences but please discuss/negotiate these with them If you have specific issues – see your Student Support Officer or Personal Tutor
