He has worked with over 850 companies and contributed to develop various innovative products and services such as Curves Japan, the world’s largest fitness chain for women in Japan, Raku-Raku phone, the best-selling age-friendly mobile phone, and the first college-linked retirement community in Japan. He serves as Professor at Smart Ageing International Research Center (SAIRC) at Tohoku University in Sendai, Japan. Not only created the concept of “Smart Ageing” but also he contributed to establish the Smart Ageing Square, the ageing-business-on-campus with various industry partners and the Smart Ageing College, the intergenerational school on the campus. Both are the first implementation in Japanese national universities. He is an entrepreneur, business producer, gerontologist, and best-selling author of several books, including The Business of Aging: 10 Successful Strategies for a Diverse Market, he Successful Business of Ageing: turning the super-aged society into business opportunities. Dr Chua Hong Teck, Director, Healthcare & Low Income Household, Performance Management and Delivery Unit (PEMANDU), Prime Minister’s Department, Malaysia Dr. Chua has about 34 years in the public service, including serving in Hospital Kuala Lumpur, Ministry of Health and the Treasury. His last position before joining PEMANDU was as Director Social Services in Economic Planning Unit in Prime Minister’s Department. Dr Chua obtained his PhD in health economics and financing from University of New South Wales, Sydney in 1998. In PEMANDU, he is responsible for the implementation of initiatives under the Low-Income Households (LIH) National Key Results Area (NKRA) and Healthcare National Key Economic Area (NKEA). Under LIH NKRA, the initiatives are to improve the their income and living standards by providing opportunities to participate in income generating programmes. For Healthcare NKEA, he facilitates the implementation of the Entry Point Projects (EPPs), covering medical insurance, healthcare travel, industry sponsored clinical trials, pharmaceuticals, medical devices and aged care. PEMANDU organised a lab in 2012 on seniors living or aged care to develop retirement villages, home care and mobile services, transformation of institutional care and the development of eco-system for aged care facilities and services. His other areas of interest include healthcare financing and reforms. Professor Alfred Chan, Chairman, Elderly Commission, Government of Hong Kong SAR Prof. Chan has been both a practitioner in welfare services for older persons and an academic in social gerontology. Starting his career as a nurse and later on as a social worker in serving older persons, Prof. Chan has extensive skills and knowledge in health and social care services and policy making. His academic interests, such as the interpretation of intergenerational relationships, ageing and long-term care policies in Asia Pacific, the development of health and social care measurements, Quality of Life, Caring Index, etc. are closely related to this area and his research have been widely published in refereed journals and as book chapters. He is currently the Director of Asia-Pacific Institute of Ageing Studies and Office of Service-Learning, which he founded in 2006 in the promotion of “Liberal Arts Education” to university students through “Serving-to-Learn; Learning-to-Serve.” He has recently been appointed by Tsao’s Foundation of Singapore and National University of Singapore (NUS) to be the Advisor for their International Longevity Centre (ILC) from June 2011. In recognition of Prof Chan’s invaluable contributions to the community, he was appointed as Justice of Peace (JP) in 2001, and was awarded the Bronze Bauhinia Star (BBS) by the HKSAR Government in 2006. Mr Daniel Reingold, President & CEO, The Hebrew Home at Riverdale, USA Daniel Reingold, M.S., J.D. is President and Chief Executive Officer of The Hebrew Home at Riverdale, in New York City. The Hebrew Home is a non-sectarian, not-for-profit geriatric service organization which provides residential health care, senior housing communities as well as a full spectrum of homecare, rehabilitation, and adult day and overnight respite programs offered by its ElderServe community services division. The Hebrew Home serves more than 10,000 older people in the Bronx, Manhattan, and Westchester County. In 2005, The Hebrew Home at Riverdale established The Harry & Jeanette Weinberg Center for Elder Abuse Prevention, the nation's first comprehensive elder abuse shelter. Mr. Reingold serves on the boards of directors of the Continuing Care Leadership Coalition, the Greater New York Hospital Association and the Association of Jewish Aging Services. Mr. Reingold received a B.A. from Hobart College, an M.S. from Columbia University, and a law degree from the Benjamin N. Cardozo School of Law of Yeshiva University. Prior to joining the Hebrew Home in 1990, Mr. Reingold was an attorney in private practice representing non-profit organizations.

Applications that are incomplete or do not meet the criteria will not be qualified for submission. 3.Each award category entry must be contained within 1 Page only. Please do not use all BLOCK Letters. The following format is to be used: Only ‘CAPS’ the first word of each sentence. Font size is 8, Font type is Arial. 4.Please include any photos in the awards application form, and email copies of the photos in high resolution, min 300 dpi, jpeg file to awards@ageingasia.com. You can email the files via dropbox.com link or www.wetransfer.com 5.All video file submissions should be in MP4 format, not exceeding 2 minutes. A video submission will be a good supporting document, but it is not compulsory. However, the “Facility of the Year” award category requires a compulsory video submission - Please label the video file under the facility name. Award application is only accepted upon receipt of full payment of entry fees. 8.There is an entry fee of S$550 per award category. The first award submission is free for Ageing Asia Alliance members. 9.The submissions will be reviewed by the APAC Eldercare Innovation Awards 2015 judging committee. 6 finalists per category will be selected by the judging committee. 10.Finalists of each category will be announced on 27 February 2015. Live voting for finalists will commence in March. 11.For the final results, the judging committee will influence 60% of the votes and 40% will come from by the 6 th Ageing Asia Innovation Forum (AAIF) audience participation. Only registered participants of the 6 th AAIF are eligible to vote. Each participant is only allowed to vote once. Voting closes on 15 April 2015 at 2pm. 12.Finalists will have the opportunity to showcase their submissions at the 6 th AAIF. 13.The organisers reserve the final rights to publish only eligible award entries in the final awards booklet. 14.Finalists will be featured in the awards’ marketing collateral.
