1 Wiltshire Pupil Health & Wellbeing Survey 2015 This survey Enables the voice of children and young people from across Wiltshire to be heard Involves Years 4, 5, 6, 8, 10 and 12 Takes place every 2 years Is anonymous, secure and confidential The results help schools provide PSHE education and pastoral support to address pupil needs Helps schools monitor the impact of work, based on feedback from young people Informs Policy and Practice a ‘You said, we did’ approach Informs local Commissioning and the targeting of local services and resources for young people

7  There are some questions about where you live and your home situation  For example about which council district you live in e.g. Amesbury, Bradford on Avon, Calne, Chippenham, Corsham, Devizes, Marlborough, Malmesbury, Melksham, Mere, Pewsey, Salisbury, Southern Wiltshire, Tidworth, Tisbury, Trowbridge, Warminster, Westbury, Wootton Bassett or Outside of the County  There is a question asking if either of your parents are in the armed forces. 8 This means you cannot be identified. Your answers are extremely important They will guide the way Wiltshire Council spends money on young people over the next couple of years They will be used to help the school understand more about you It’s very important you are honest and sensible Your answers are anonymous Wiltshire Pupil Health & Wellbeing Survey 2015

9 Healthy Eating/ Physical Activity You will be asked if you would be interested in eating breakfast at school if it was provided - this is for example through a breakfast club You will be asked if you are eligible to have free school meals For the physical activity question, the definition that is used is an activity that ‘makes you breathe harder and faster than you normally would ‘ Wiltshire Pupil Health & Wellbeing Survey 2015

10 Alcohol For the question ‘Do you drink alcohol?’ This refers to a proper alcoholic drink - a whole drink, not just a sip (Please don't count drinks labelled low alcohol). Wiltshire Pupil Health & Wellbeing Survey 2015

11 There are questions about illegal drugs, so called “legal highs” and prescription drugs. The questions about prescription drugs are those that are NOT your own & medicine you can only get from a doctor - not aspirin etc. Illegal drugs Wiltshire Pupil Health & Wellbeing Survey 2015

14 There is a question about whether you need more knowledge about keeping safe from grooming “Grooming is when an older person starts a relationship with a child in order to sexually abuse that child” Remember this is important.
