2 Recovery Support Systems Purpose: The overarching goal of this discussion is to: a)expose you to the utility and benefits of the electronic platform in supporting the continuity and coordination of client recovery supported services; and b)provide a better understanding of the electronic framework used to document, monitor, and improve recovery support related outcomes using better care collaboration.

3 Case Management & Electronic Record Just like all other areas, Recovery Support is driven by continuity and coordination of services; as well as, accurate data collection to meet the needs of the clients and monitor the overall service program model while adhering to federal mandates regarding client confidentiality.

6 Case Management A case management electronic record (ER) is a systematic approach in the collection of electronic recovery support and case management services rendered to individual clients or populations. It is an electronic record which has the capacity to share and exchange client information across various systems of care. Using an ER allows a case manager to receive client information in real time, which results in a seamless approach to effective continuity and coordination of care.
