15 15 Forms Purpose Tells me who is representing your team Verifies parental approval of child’s participation Provides medical information should an emergency arise Agreement with BDPA policies and responsibilities (Pres, Coordinator, chaperone, students)

16 16 Planning to send a team in 2009 Final Due date for Forms is July 15 th If you are working with BETF, keep me in the loop If I don’t hear from your chapter, it means you do not plan on having a team Missing information is due no later than July 25 th Special circumstances will be reviewed on a case by case basis
