Learning Disability Strategy ‘A Good life for everyone’ Action Plan Refresh Becky Hale Service Manager – All Age Disability
Workshop A workshop took place on 15 th May 2013 for Partnership Board members to go through the Learning Disability Strategy Action Plan: To review how we are doing for each of the areas in the strategy. To sign off actions which have been completed so far.
The Learning Disability Strategy areas are: 1.Choice and Control 2.A Fulfilled Life 3.A Place to Live 4.Good Health 5.Keeping Safe Taskforce 6.Seldom Heard Groups 7.Supporting Family Carers
Choice and Control What have we done since 2011? Started the Self Directed Support Process and produced information for people with Learning Disabilities and family carers. Started giving personal budgets for people with Learning Disabilities. Self directed support forms changed so that they are more accessible on the request of PB members.
Choice and Control Continue the work happening to: Talk to providers to develop more services people would want to buy. More help for people to make choices. Better monitoring of quality of the services providers deliver.
A Fulfilled Life What have we done since 2011? Involved customers and family carers in the move away from traditional day services. 6 community hubs opened across Warwickshire offering advice and information. 4 new high support need services opened across Warwickshire.
A Fulfilled Life Continue the work happening to: Review each of the community hubs involving peer reviewers. Raising awareness of the Resource Directory. Provide regular updates about services to customers and family carers (LDPB Newsletter and website).
A Fulfilled Life Continue the work happening to: Monitor and review work with people to promote independence through the Quality Assurance Panel. Provide Changing Places to access in local community. Agree and plan the Transitions Project.
A Place to Live What have we done since 2011? Developed housing options for people for eg. Extra Care and supported living. Worked with providers who want to de-register. Developed easy read tenancy agreements booklets for people with Learning Disabilities & tenancy guide for providers.
A Place to Live Continue the work happening to: Find out the numbers of people interested in supported living and working with providers to make this happen. Give people with learning disabilities and their families, and providers information about housing support. Look at joint working with Coventry City Council on the Shared Lives Scheme.
A Place to Live Continue the work happening to: Evaluate the KeyRing Scheme. Develop extra care housing for people with Learning Disabilities. Increase use of assistive technology within supported living. Review all Out of County Placements.
Good Health What have we done since 2011? Health checks have been carried out for people with Learning Disabilities. Annual dental check plan developed to improve access to dentists – includes 21 individual programmes Training delivered to raise awareness with providers and health staff about the health of people with Learning Disabilities. Acute Community Liaison Disability Nurse recruited for South of County, as well as North of the County.
Good Health Continue and build upon the work already started: Increase the number of Health Action Plans and access to health information. Make sure people have good access to dental services. Carry on the work happening around the Winterbourne View Review action plan.
Keeping Safe Taskforce Group What have we done since 2011? Safe Places Scheme developed – Leamington, Warwick, Rugby and Stratford. E-learning hate and mate crime handbook developed for Warwickshire County Council and Provider staff to use as a training tool. Peer Review Programme developed to review Learning Disability services. Raised awareness of hate crime during Learning Disability Week 2011 & 2012.
Keeping Safe Taskforce Continue the work happening: Safe Places Scheme to be rolled out in Nuneaton and Bedworth areas and then North Warwickshire. Monitor and review contracts with providers and review any concerns. Peer Reviewers to review Learning Disability Services. Raising awareness of keeping safe and hate crime to people with Learning Disability, providers and public.
Supporting Family Carers What have we done since 2011? 3 rounds of Carers briefing meetings taken place at each of the day services. Carers involved with the tender process for high support needs service. Family carers involved in the peer review training programme. New carers assessment process rolled out.
Supporting Family Carers Continue the work happening to: Deliver information about personalisation to family carers. Involvement of family carers in co-production. Review new carers assessment process. Involve carers in the Respite review.
Seldom Heard Groups What have we done since 2011? Involved in the service redesign of Learning Disability services – for example high support needs service. Self Directed Support process made more accessible. Peer review programme developed. Learning Disability Champions involved with the development of the Safe Places Scheme.
Seldom Heard Groups Continue the work happening to: Check to see how many people from minority ethnic communities are using our services Parents with learning disabilities have access to all services and support to make sure they can continue their caring role. Peer reviewers to spend time with seldom heard groups and to see what they think of services.
