5 What are the roles of school and district based staff? At the time of Enrollment Parent completes the EH40, Home Language Survey The school secretary will schedule enrollment center to set up a time for the student to be evaluated for ESOL eligibility

6 What is the process ? The Enrollment Center staff will administer the W-APT W-APT Indicates a Performance Level Score of 4.5 or lower = ESOL placement Enrollment Center will enter data in SCN ELL Transcript Analysis will be done at 440

11 Exit Criteria How do students exit from the ESOL program?

12 ELL Data Folder Maintained by school based ESL teachers Records, including the W-APT results, initial Parent Notification Letter, ACCESS for ELL results, a copy of Exit Notification to Parents, and the Year 1 and Year 2 Post-Exit Monitoring forms must be included in the folder.
