2. Safety Integrity Teamwork Excellence Background An ACES contract employee was performing inventory in B1194 at the time of the injury. A computer tower weighing approximately 100lbs began to tip and the employee caught it, injuring their shoulder. The employee reported to the NASA Clinic one week later after missing 5 days of work. The Clinic referred the employee to an orthopedic specialist. However, the employee resigned their position the next day and there was no follow-up visit.

3. Safety Integrity Teamwork Excellence Investigation The supervisor said the employee was working alone at the time of the incident, and no one knows exactly what happened. The supervisor did not know why the employee waited a week to report the injury. No further investigation was possible, as the employee had resigned and was no longer available.
