2 Lockout/Tagout/Verify Procedures b The purpose of lockout/tagout/verify is to prevent energy from accidentally being released while a machine or equipment is being serviced. b The ultimate goal of lockout/tagout/verify is to protect the safety and health of employees. b Secondary is the protection of equipment from damage.
3 Definitions of Terms You Will Likely Hear : Energy Source: --Any Source of Electrical, mechanical, hydraulic, pneumatic, chemical, thermal, gas, water, steam, air or gravity.
4 Isolating Device b A mechanical Device That Physically Prevents The Transmission or Release of Energy. --disconnect switches, slide gates, valves, blocks, and blind flanges. Examples are: E-Stops are not included in isolation devices.
5 Equipment Specific Procedures Specific procedures for : v shutting down the equipment. v isolating from its energy source. v blocking from residual energy source. v securing equipment for work.
6 Lockout/ Tagout b The placement of a lock and tag on an isolating device which ensures that the equipment being controlled cannot be re- energized until the locking device is removed.
7 You Will See Different Colored Locks. b Blue locks will be equipment locks Used to protect equipment.Used to protect equipment. b Red locks Used for SGS Personal Protection.Used for SGS Personal Protection. b Green locks Given to contractors to use on SGS equipment.Given to contractors to use on SGS equipment. b Orange Locks Group locks for person controlling a group lockout.Group locks for person controlling a group lockout.
8 You Will See Two Different Color Tags In Use b Red Tags With Red Locks For Personal Protection Of SGS Employees. b Red Tags With Green Locks For Contractor Protection. b Yellow Tags With Blue Locks For The Protection Of Equipment. v Tags Function is Communication. vWhy They Shut The Machine Down. v Tells Who Shut The Machine Down.

12 Occasionally You May See a Yellow Tag Without a Lock On a Piece of Equipment That is Out of Service. This machine is Tagged Out because it Will Not Run. The Tag Will Tell You What Is Wrong With The Equipment. Never Attempt To Operate Equipment That Has Been Tagged.
13 Verify b The process of operating the start controls, engaging levers, measuring voltage, inspecting lockout devices valves,disconnect switches, blades, piping systems in an area to make sure that all energy sources have been isolated and controlled.
14 You Have a Role b You are either an Affected Employee b An Authorized Employee b Or Both !
15 Authorized Employee Authorized Employee-an individual who uses the lockout/tagout /verify procedure on a machine or piece of equipment that is to be serviced or maintained.
16 Affected Employee b Affected Employee-an individual whose job requires him/her to operate or use a machine or piece of equipment on which servicing or maintenance is being performed under the lockout/tagout/verify procedure. This also pertains to any employee whose job requires him/her to work in an area where servicing or maintenance is being performed.
17 You May Be Both An authorized employee and an affected employee may be the same person, when the affected employees duties also include performing maintenance or service on a machine or equipment that requires Lockout/Tagout/Verify procedure.
18 Key To Successful Lockout/Tagout/ Verify is: Everybody working together.
19 Authorized Employees that: b Properly plan the job. b Notify all affected employees in the area of work. b Shut down the equipment at the operating controls. b Isolate all energy sources to equipment. b Lock and tag all isolating devices b Dissipate all stored or residual energy sources. b Verify the isolation.
20 Affected Employees that: b Stay clear of the area as much as possible. b Never attempt to assist. b Never interfere or tamper with a lock or tag. b Report all unusual situation to your supervisor or foreman.
21 As an Affected Employee: b Instrumental in helping protect the safety of yourself and your co-workers. b May save someone or yourself from grave injury or even death.
