2 Overview You can now process wellness claims for policyholders during open enrollment. You can submit the wellness claim directly to Aflac’s Claims Payment System from the Associate Services Web site.

3 Training Objectives Provide overview of the wellness system.  What does the Submit Wellness Claim page look like?  What will you see?  How is the system accessed?

4 Provides you the ability to submit a wellness claim for a policyholder during open enrollment. Provides a list of wellness-eligible policies available for each policyholder. Provides you with step-by-step instructions to complete mandatory fields on each screen. Creates a claim history accessible within CSI. High-Level Functionality

5 Usage Requirements You must have access to Associate Services. You must have Internet access. You must have a signature pad.

6 Associate Services Access When you are with a policyholder during a group open enrollment, you can access the wellness system via Associate Services (Grow My Business > Technology > Policyholder Service Tools > Submit Wellness Claim). The Submit Wellness Claim link is located under MobileAflac sm.

7 Online Submission: Step 1 of 5 Once you have selected Submit Wellness Claim, you will be taken to Step 1 of 5 to begin the claim filing process. From this screen, a series of customer identification fields must be completed to include: Policy Number or SSN Policyholder’s First Name Policyholder’s Last Name Policyholder’s Date of Birth Policyholder’s ZIP Code Note: You can cancel the process at any time; however, you must select Next Step on this page to continue through the process.

8 Online Submission: Step 2 of 5 Note: You can cancel the process at any time; however, you must select a policy from the Policy Number list to process a claim. Under the policy selection list, policies that are currently available for the policyholder are dynamically populated for selection. In Step 2 of 5, you will verify the policyholder information and will enter or make updates to the policyholder’s Personal E-mail Address (if necessary). You can quickly reference the policy verbiage regarding wellness eligibility in this section. If the policyholder has filed a Wellness Benefit in the last 14 months, the Date of Last WLB field will reflect the last WLB treatment date.

9 Online Submission: Step 3 of 5 Note: You can cancel the process at any time; however, you must select Next Step on this page to continue through the process. On Step 3 of 5, you must complete each of the required fields in the Patient Information section. The E-mail Address field is optional on this screen. You must also select a Wellness Benefit from the drop- down list and enter a service date. The benefits listed in the drop-down box are specific to the policy selected. The Provider Information section must be completed in its entirety before proceeding.

10 Online Submission: Step 4 of 5 Note: You must select Submit on this page to complete the process. In Step 4 of 5, you will verify the information previously entered is correct. If something is incorrect, you can select Edit Claim Information to make necessary changes. If all information is correct, you will obtain the policyholder’s signature using the connected signature pad by clicking on the Sign button. Once the signature is obtained, you must click on Submit to send the claim to Aflac’s Claims Payment System. The fraud statement must be read and agreed to by selecting the radio button.

11 Submission Complete: Step 5 of 5 This is the confirmation screen, indicating a complete submission and the final step in the process.

12 An e-mail is sent to the policyholder once the wellness claim is submitted–if the policyholder provided an e-mail address during the process.

13 Review 1.The new wellness processing system is located on the Associate Services Web site (Grow My Business > Technology > Policyholder Service Tools > Submit Wellness Claim). 2.Customer identifying fields at login ensures that the wellness claim is on the correct policyholder. 3.Policy information clarifies wellness eligibility for the policyholder and associate.
